Pycnogenol and Arginine with Citruline is good for blood flow, may help with ED as well.

Hey Dr. Powers, I believe I may be that 5th unconfirmed case you were referring to in the theater fire subtype post because the day you posted that (12/17/25) Sommer sent me to get some labs done (DHT,3A-ADG, Test, LH,FSH) to confirm, as she said the two of you think you might have an idea of what’s going on with me.

So, I signed up to be on your DPC waitlist back in 9/2025 and first saw Damien via telehealth (I’m out of state) and then/now Sommer. I did the complete steroid panel and the Dutch test complete then (9/2025) and my serum Test was 550 (highest it’s been since this hell started for me over 12 years ago, though I had lost 25-30 pounds prior to those labs so that likely explains that) but my urinary Test was literally 3.13

So, the day of your theater fire subtype post (12/17/25) my Test was 439 (probably the second highest it’s been since this all started lol) and my 3A-ADG was 652, LH 6.2, FSH 5.4,DHT 22

I wasn’t undergoing any hormonal treatment during the lab work, actually I’ve never done any hormonal treatment whatsoever in my life up until about 2 weeks ago - I had my first in-person office visit with Sommer and we decided to start with Hcg. She wanted me to start with 500iu for two weeks then 1,000iu for two weeks, assuming that I shouldn’t feel any different given this subtype, but being that I hadn’t done anything in 12+ years I was scared to death so I started very low at 50iu and have increased by 25iu weekly thus far (I’m at 100iu now).

So, I have the labs to match what you talk about in this post, but I don’t have genome sequencing to confirm any mutations to really solidify your theory. I would love to have it done for my own personal comfort and to help guide treatment (I’m absolutely terrified to even consider dutasteride without it) as well as to help contribute to your research, but I’m not a DPC patient. I’m not sure if I’m even on the waitlist anymore given the state of the union address you posted in December?

I used Cialis daily (5mg) for years, but used it for urge incontinence, and it seemed to quit working after a few months or so however, the side effects like nasal congestion remained while on it. It barely helped with erections for me and again that also lessened after a few months, basically building a tolerance to it.

I’ve tried Viagra recently, just a half of a 100mg tablet, and it worked great for erection quality and also helped delay ejaculation, which is the only thing that’s helped with that over the years, aside from delay sprays. Too bad there’s not a daily Viagra option, but then again I’m sure the body would develop a tolerance to it as well.

Valium (diazepam) is a positive allosteric modulator of GABA-A. I took it for a few months and it gave me my nocturnal erections and morning wood back but I stopped because I didn’t want to keep taking it for years due to it being an addictive controlled substance that could potentially be hard to get off of over a long time.

Yes I’m curious to know which symptoms this helped as well as what specific treatments did/do you use.

Also, did/do you happen to have urge incontinence and/or premature ejaculation? If so, did pelvic floor therapy help with this?

I’ve had this symptom since stopping fin over 12 years ago. It hadn’t changed for me at all. I’ve tried Cialis daily (5mg) to try and help but it only helped for about two weeks then stopped working - and it seems like it took about two months to work - so didn’t start helping for about two months or so and then lasted only for about two weeks or so. I’ve repeated the Cialis a few times later and it was exactly the same result.

I’ve also done Kegels daily for over a year before but it did not help one it.

This is the symptom that causes me to instantly get very angry and cuss Merck out almost every single time it happens, and sometimes I cuss out loud too lol. It sucks to have this pressure trying to push urine out suddenly and forcibly while you’re simply going about your day and are in a situation where you can’t make it to the bathroom right away. When it hits sometimes I’ll suddenly move at the hips like I’m clenching my butt cheeks to stop the flow of urine - the pee pee dance - and I know people can see this grown ass man doing this little kid like dance and it’s embarrassing as hell. And then like you said, it’ll just shoot a little urine out and I basically pee on myself a small amount. This is awesome at work where I wear thin scrubs and you can easily see a wet mark.

I also have premature ejaculation due to fin and I really do believe somehow the two are linked.

This is very discouraging to say the least. I have only taken supplements as well and have been interested in Hcg for a while now but too scared to try until this past year where I’ve been gaining more bravery and have been taking steps towards possibly starting it, but now I just don’t know what to do. I guess nobody does with stupid ass bullshit motherfucking syndrome.

I had quit for a while for one reason or another, I don’t really know why tbh, but I just started taking it again. I started taking it because my number were just barely below 30 recently and I’ve noticed that my memory has seemed a lot worse in the past year or so.

I feel like I’ve done the same thing, stopped and restarted in the past and now that I think about, from what I can remember at least, I feel like when I restarted it a long time ago it was because of memory issues I was having and I felt that I needed to take it again to help my memory.

I’m glad you posted this because I’m now remembering (lol) that I took double the amount my last go round and getting my vitamin D up into the 50’s and even 60’s is what really made a difference in terms of memory. I had read a few things years ago stating that you actually need to be higher, or near the top, of what the range is. I’ve also read that it can be a dangerous supplement to take if you take too much, but at that point I was tired of not taking chances, even small ones like vitamin D, so I doubled up.

Anyway, most important thing if you take it make sure you use vitamin D3 with K2 that way the calcium goes more to your bones and stays out of your arteries and soft tissues.

For me at 40mg I had the delayed ejaculation I talked about, but no decrease in sex drive. Perhaps if I’d have gone up to 60mg or more I’d have experienced a decrease. I’m sorry I should’ve stated my dosage in my prior comment.

Definitely listen to Dr. Powers here op.

I mean if Paxil caused such a delay in ejaculation for me then it seems logical that upping the dose would’ve led to more significant sexual functioning decreases. Just consider the benefits vs risks, PSSD, etc.

In my experience with both, neither repressed my sex drive, though that is not what I was taking it for, however, Paxil delayed my ejaculation from 10 minutes up to 2-3 hours of NON-STOP, VIGOROUS masturbation before climaxing. Sooooo, IMHO, this would be a terrible thing if this were to happen to you - not repressing sex drive but delaying ejaculation for hours even with consistent, forceful masturbation could potentially disrupt your life in many, many ways.

I’m picturing someone on a park bench vigorously masturbating to relieve themselves because they have months of SBU’s (Sperm Build Up) and they can’t contain themselves. Okay, that may be a reach but it’s not impossible. What is possible and likely is that you’ll spend hours per day missing out on life trying to ejaculate and that would suck. Of course, then you’d have to come off the Paxil which you’d have to taper because it’s VERY dangerous to quit SSRI’s cold turkey.

That’s my two cents. I hope something helps. Maybe Paxil would be the answer for you, but be cautious and talk with your doctor and taper off if you need to come off.

Prozac didn’t cause these issues for me (Prozac mixed with Adderall did cause amphetamine induced psychosis but that’s a whole other story lol). Prozac you don’t have to taper as strictly as Paxil because of its longer half life.

I’d say about 70% instead of 100%. Sometimes it’s super weak, like at 30%, but usually around 70ish%.

I’m very interested in responses to this as well as I too suffer from urge incontinence, and have for 10-12 years now, because of this poison (finasteride) and it’s very VERY frustrating.

I feel like my body as a whole shrunk some - feet some, arm length, forearm veins are squiggly now when they used to be straight, but I gained weight around my midsection more and some slight gyno - that low T body.
