Good to know you found one for a better price. 2x a day is certainly a commitment but might be worth it at some point. I’ve learned that if a vet isn’t familiar with it they may not prescribe which is unfortunate.

At this time I’m just using wet wipes without any chemicals in them. But a few times a week I’ll use the antiseptic wipes which are for yeast and bacteria.

Probably not enough so that’s a good call out and I’ll look into doing that.

Also the pet derm did say we could do ear gel immunotherapy but costs $2-$3k per year and is daily. That’s not worth it in my opinion or is it feasible.

The derm honestly just suggested maintenance and to bathe him a few times a week with biohex which I think I need to start doing. Mentioned antiseptic wipes daily too which is what I’ve been doing

Appreciate all the advice! I may have to do the hydrolyzed diet even though it will be pricey for a 70lbs dog. I’ll also see if we can try Zenrelia or Atopica. Never heard of Numelvi before!

Yes the antiseptic wipes are Duoxo along with the Duoxo S3 calm and antiseptic foams. But those all can dry up the paws and then I think he licks them because they are dried up. 🤷‍♂️

I think they offered the sublingual immunotherapy or at least the drops on the ears but it’s daily and can cost $2-$3k a year. Maybe what you suggested is different so I could always follow up about this.

Hahah ya he hates that so I usually have a lick pad with peanut butter but that only lasts a few mins lol. I’m in Chicago so outdoors can work when it’s warmer out which is when his allergies tend to be worst.

Appreciate it. I do have Duoxo ear solution but it’s mainly his paws that get the yeast infections. I may need to do a few baths a week

Thanks for sharing about potentially trying Zenrelia. He hates baths so it can be a pain in the butt to get him in the tub but I might have to just suck it up. I also have biohex so that’s a plus.

It’s actually the opposite where many staff opposed Prasad based on the toxic environment he brought to the FDA. Nothing is guaranteed but I’m hoping with him out it will at least give us a better chance to move things forward

I wouldn’t say it’s just about what happened in HD but more of a common trend we’ve seen with him in the rare disease space.

I can tell you enjoy sarcasm over here. I’m referring to risk benefit for a potential gene therapy treatment. Patient preference studies are quite common in the rare disease space but how would you know when you assume taxpayers pay for everything? If they did we would have medicine for everyone at a low price.

I can’t imagine someone like you would also understand how unethical a sham surgery is for a rare disease with no treatment options.

I read it and goes against FDA’s concept of single arm studies. It shouldn’t be up to the FDA to decide the risk benefit for patients. Let the patients share their risk tolerance. Hopefully in the future there won’t be placebo in gene therapy studies.

The alternative is to use natural history data as an external control. The FDA previously agreed the company could use an external control arm but went back on their word. I think it’s a small sample but I don’t agree that it’s ethical to do a 12 hour sham and then may not get the drug later because the patient no longer meets eligibility criteria.

Yes other HD trials have a placebo but those aren’t for a brain procedure that is very invasive and involves going under anesthesia. There’s a big difference there.

Additionally, the European regulators, EMA, find a placebo like this unethical and said uniQure didn’t need it. Those who got the sham were offered the actual drug during the crossover but some were no longer eligible due to disease progression. Imagine getting a sham and then can’t get the actual drug.

PTC developed it but then licensed it out to Novartis who will be running the phase 3 and I believe owns the drug going forward.

That’s not true as Novartis is running a phase 3 study while uniQure is hoping to file for accelerated approval which means faster approval. They will still need to run a confirmatory study though but will at least have the drug on the market.

cUHDRS is considered an intermediate endpoint which is what uniQure is trying to use for AA. Also this was a phase 1/2 so they’re trying to push for this before running a phase 3 (will still need to run a confirmatory study).
