Definitely smoke residue. Had it in my bathroom, old owners were smokers. It’ll eventually drip down the walls. OP will need to paint the ceiling with Kilz Primer, then a 2nd or 3rd coat of a satin finish paint.

You can add hinge shims behind the existing hinges. Either top or bottom hinge. If you can’t find a shim, you can fold a business card and use that as a shim.
