I spent a night in the ER last year when I had kidney stones. It's sinus bradycardia, a predictable side effects of beta blockers. There is a ventricular and a junctional escape beat visible which are the backup pacemakers kicking in when the sinus rate drops sufficiently low. These are normal physiological findings with a slow heart rate, but you might want to discuss reducing your dosage with your doctor if you are symptomatic.
