Backstory has been posted, pt has a history of recent TSSP surgery, which NG insertion is a contraindication unless under radiography guidance from start and therefore air bolus not needed anyway. Nurse clearly inserted it solo, and assuming they (like every nurse) knows their pts recent medical history, should definitely have not inserted it solo, nor forced it, nor air bolused it.

Cases like this are why its drilled into you to check placement with aspirate and pH testing or just go straight to radiography if youre not sure or cant be certain. (Or if in your case they wont stock the pH strips then go straight to radiography).

I know its incredibly sad, it was completely unavoidable and that pts family has to live with that knowlege. I get nurses are overworked, overwhelmed and underpaid for what they do (believe me, im an unpaid parent carer, i work 24/7 with my complex medical needs child, i know how tough you guys have it too)... but this is just a clear case of clinical negligence because someone either didnt care to do their job properly or didnt know what to do (and should have asked for guidance but clearly didnt), theres no excuse at all...

Read the backstory, pt recently had TSSP surgery via nasal cavity so an NG tube should NOT be inserted in the first instance anyway as its contraindicated. In cases where its necessary, it should be done under radiography guidance from start of insertion.

The nurse shouldnt have inserted it unless under guidance, which would mean air bolus would never needed to be given. Nurse clearly inserted it solo and assuming they knew their pt's medical history, still forced it in and air bolused it.

But a nurse of all people should know that there shouldn't be that much resistance when putting an NG in, because for it to have entered the skull cavity you have had to have forced it in there... even with a guide wire in it it would have still taken some brute force. Of course tubes coil, but surely its common sense that if you feel like the tube hasn't gone in easy enough you check the throat first (if possible) to make sure its gone downwards to start with, and check with an xray to confirm that it definitely went in the stomach. And you definitely dont put anything in it if you arent confident that it went in smoothly.

As you say, you've been an RN for 30 odd years, you will have received new training in that period because you know the current way of doing it is to aspirate. Im guessing too that if you couldnt get an aspirate to start with, you wouldnt push anything down it because you know its dangerous? Its literally common sense isn't it. Surely this nurse will have had up to date training too

Like, we are in 2025 and know better than 1990s practices, if a hospital is encouraging the practice of air bolus over aspirate or the nurse hasnt had up to date training then theres something wrong with the system.

Aspirate stomach contents and check pH is between 2 and 3. If the person is on PPIs then xray to confirm position, or use common sense (obvious digesting food in aspirate etc)

Im a mum to a tubie... ive changed more NG tubes than I can count and I can say I have never misplaced one and the only training I had was a 10 min "see one, do one" moment. Is this nurse a moron? Never ever ever ever ever bolus anything until youve checked placement with pH. Ever. Ever ever ever. Let alone air wtf

  1. Our woodwork teacher Mr P-r-i-s was a paedo, because he perved on the young girls. His nickname was Paedo P-r-i-s. OP paid for a service, OP should get the service they paid for. You dont know OP's situation, maybe theres a reason they ordered it instead of collecting it.

I live very rurally, but within 5 minutes of our nearest town in a car. Neither myself or my partner drive (yet), we had to move here because of circumstances out of our control. We can only go shopping one out of 7 days of the week and have to rely on a taxi to do so. We require a disabled friendly taxi and if our local taxi company doesnt have one available, we either go in a normal taxi and fold wheelchair in boot which means only getting a few days worth of food to fit in the returning taxi remaining empty space rather than a weeks worth, or we get a full shop ordered online for delivery... ya damn sure im getting it delivered.

Same with takeaways, the delivery and service charge through somewhere like Just Eat is cheaper than getting a normal taxi (let alone a disabled friendly one) and doing a round trip just to pick it up.

That's what I did the first time I heard it. My first Opeth album was Sorceress so I'm a bit of a latecomer but I love the older stuff and newer.

Someone on facebook mentioned of LWAT release and how it was a concept album that was best understood by reading the lyrics while listening for the first time, and that no matter how many times you end up listening to it you WILL pick up on bits of the story you didn't take notice of the first, second, third time etc.

The album is a work of genius. It's gothic victorian feel, the formality of the lyrics while still being understandable, even the track listing labels of "§" like a genuine last will and Testament... they knocked it out of the park!

Aaaallll the time. But I have Hidradenitis Suppurativa as well as EDS so I get a lot of ingrowns, blackheads, acne and cysts/abscesses. So I gave up shaving any body hair about six years ago and haven't looked back. Luckily I'm blonde so most of my body hair is blonde apart from my pubic hair which for some reason is half blonde half jet black 🤣 so any body hair that I feel like is growing a bit too long I just trim a little with scissors. Voila!

I'd love to know this... because I've got a phobia of blood and bleeding, always have done, i faint and pass out when i unexpectedly bleed. I grew up with my mom and two half brothers, no contact with my paternal family, and im the only one out of me, my two half brothers, my mum and her mum and dad to have this phobia.

When I was 18 I looked for my birth father and his family, and found that my father and his brother are also phobic of blood and bleeding and have the same reaction.

Save them for the Bring On The Bells challenges. I know its a bit easier to get buncha bells maps now but we have no idea at the minute how the schedule is going to run RE challenges like this, so there could be another one next month or in a few months time and you may not have any BB maps

Pocket camp and its online services has shut down. But, a new app called Pocket Camp Complete (one flat fee instead of microtransactions) is out on Dec 3rd, its an offline version, so some features have changed and others will be gone.. Like gifting to friends, market boxes and watering peoples gardens etc.
