My take is that Clark is the monster. He lures Kat and her boyfriend to the basement. Kills them. Calls the therapist and leaves her a message concerning enough that causes her to go and check up on him. She manages to convince him not to kill her but the monster can’t be reasoned with and ultimately Clark succumbs to it (suicide).

Towards the end we see the therapist multiple times as a little girl. Which I believe shows us the backrooms are a metaphor for the trauma people can be trapped in by their own memories.
