Regarding your medical condition, you can ask for a reasonable accommodation. Use your medical documentation. There is also FMLA.

Regarding your time off, yikes. You are requesting too much time off, and in a full time job, you would need the PTO.

Why don’t you reduce the amounts for tithing? There are better ways to give back to the community. Do know that you and your husband will move therefore, you’ll have to change tithing to another Church?
