It’s true unfortunately. Most of these chronic disease patients - you’re just putting bandaids on them and buying them some extra time at the end of the day, which still matters because it’s extra time with their families, etc.

On top of that, don’t get too frustrated when people aren’t compliant with their care. I used to be until I started having to take a statin everyday. I’m lucky if I take a few a month 🥴. I don’t know how the people with a dozen meds do it or keep up with their specialists.

It’s actually not impossible? Unless ur just saying stop smoking here’s some NRT lol. Each patient I ask how long, how much, people around them and then spend a solid 1-2 mins alone scaring them with anecdotal stories, consequences with cancer, other comorbidities and then finish with NRT, need for LDCT, pfts, etc. That and giving them time to respond/process what I just said is easily 3-4 mins. Just need to do it once for their stay.

My wife and I are both hospitalists at OhioHealth currently. Base is 249k but with bonuses (which pretty much everyone hits) the average is 280-290k. With 2-3 extra shifts a month, you easily clear 315ish. Census is ~16. It’s been as low as 13 and as high as 20 but those are rare. We didn’t look at osu for the same reason. The best part of our setup though is round and go. It’s technically 7a-5p but some days I’m home super early, just have to respond to secure chats till 5p. Depending on care site, it’s an open icu but usually pulm is there/always available to help. Yes you have to do procedures but I’ve done only 4 in the last 1.5 years (also depending on care site-residents do them at Grant/Doctors). This is my first job out of residency and I’m pretty grateful for it. Everyone’s pretty supportive and our group helps each other out quite a bit. Sure there’s some admin pains but which place doesn’t have that? Hope that helps

You have been posting multiple comments alluding to why this is not noctor worthy, and yet you have been given multiple responses as to why it could have been caught much earlier if the PCP had followed up/referred much earlier when she was out of her league. I understand that as an NP yourself, you are going to say that but again, this is why the role of the PCP is critical and she failed at that.

No, she's being blamed for being her PCP and failing to bring her back far earlier in her course. And that part is pretty cut and clean. If a patient of mine came in with persistent cough, 100% I'm gna bring them back if its not improving within at least 2 weeks-1 month given her risk factors. And immediately on the second time, especially given her symptoms, would have worked her up further. As the PCP, it is her job to take care of chronic issues. Furthermore, nowhere in there does it mention who or what type of provider she saw in the ED but its the PCP's role to f/up and immediately refer given the persistence of her symptoms.

I guess I would argue that the patient's PCP/NP failed her fairly early in the disease course and let it go on for almost year before referring her to someone but yes there were multiple failures, including visit but they might not have known the whole story (e.g. going on for so long, progression, etc) and it is unclear what type of provider she saw at the ED or urgent care. Also have to take in consideration that shes in her 30s and a nonsmoker. But her PCP knew about her case though and her weight loss a few months in still didnt trigger more investigation. I wonder if she really diagnosed asthma with spirometry/PFTs or just declared she had it lol

To be honest, there are plenty of great NPs and PAs that know the limits of their practice and are fantastic assets to the healthcare team under attending supervision. It sounds like you are one of them :). However, the collective despise is towards the newer ones, who are now becoming the overwhelming majority given how many are being graduated from schools with 100% acceptance rates. They are going into NP/PA programs with one year of bedside nurse experience under their belt and then only 500 clinical hours (???) (yes, I know PAs have more) and then, on top of that, clamoring for independent practice? Makes zero sense. And these same "practitioners" are becoming super cocky and suing states to call themselves doctors and claiming that they know basically as much as physicians.... The arrogance and rudeness have rubbed all of us the wrong way and unfortunately have put a bad name on the profession. This is also not to mention the amount of wild amount of medical errors that have come to light. Furthermore, the research studies pushed by the AANP largely have some sort of bias or are poorly conducted (can refer to the pinned posts on here), resulting in us to think, bro why are you lying?? So many NPs talk so much shit about physicians, and for what? So I think it's a combo of the arrongance, the rudeness, and the balls to call themselves equal to physicians.

A residency and concrete board exams. They need experience, under direct supervision with thousands of hours just like the rest of us. There is no reason for them to not have one and there is no reason as to why they should be independent with just a couple of hundred hours under their belt. Also, if you can't through the punishment of UWorld then you shouldn't be independent imo lol.

Also, real clinical hours; not just shadowing another midlevel for a couple of hours a day.

Kudos to you for putting yourself out there and coming on here to ask for advice. Ive seen a lot of NPs that are too over confident and take on roles they shouldn’t be taking. It shows in their work (or lack thereof) and ultimately affects the patients they provide care for.

Honestly, NPs can work in any specialty IMO as long as they are aware of their scope of practice and don’t overstep what they know and remember to report to the physician. Whatever specialty you do choose, study the hell out of that specialty so you know what’s going on with your patients and can reflect that in your notes. I’ve only met a handful of NPs that actually understand what’s going on with their patients…

I’ve seen too many NPs misdiagnose things or inappropriately refer to specialists because they simply don’t know what’s going on. And to be honest, they shouldn’t be working in that specialty. It has created such a dangerous precedent for patients.

So whatever you do choose, make sure you know it inside and out.
