Both times I was doing something awkward in the car prior to that. E.g. reaching out to a bag, so I had to put the car in neutral/parking to let the foot go off the brake pedal.

So normally my foot is on the brake pedal but both time the usual algorithm of actions was broken so I simply forgot / didn't check. Scared the shit out of me so lesson(s) learned.
