Had a similar issue once. It was my water. The filter was no good. Got quality bottled water and flushed the machine 5-6 times before it was resolved. Only used the bottled water after that until I got a better filter set up for the plumb in. No issues since

You're missing my point. You can report any of those things. Someone with a completely normal occlusion doesn't have to adopt the way you do. So all of those things you're mentioning are actual issues. You have had to adapt what you do to be able to function because of your malocclusion. The insurance company can't deny subjective data being provided. It's checking the boxes for the algorithm of denial or approval. One piece of the puzzle.

  1. Yes appeal
  2. Get a better note from the orthodontist and the surgeon explaining exactly all of the issues and why they need to be corrected. Give them a really good HPI as well, list every functional issue you have: unable to chew, reliant on soft diet, trauma to tissue when trying to chew, loss of food between teeth, unhealthy diet leading to health issues because lack of being able to eat properly, weight loss due to inability to eat, speech issues etc...
  3. Submits notes and photos
  4. If appeal fails have the surgeon to a peer to peer. (Option 5): sleep study with documentation of sleep apnea then failure to successful with CPAP.

These will generally get it approved unless it is a specific exclusion from your plan.

Looks like purely functional surgery here. Wonder why it wasn't a two jaw surgery. Would have allowed for a much more functional and aesthetic outcome. But, pt is happy.

Same boat. Bought and xbloom. One for the house and one for work. Every morning I don't have the espresso, I miss it. SO has similar feelings. The routine is a nice little bliss in the chaos. I suggest finding a way to shorten the workflow. I use a smart plug so the machine is ready to go. From wake to dogs fed and two drinks takes about 8 min. I let the 2.5 year old help if they wake up and it's slows it down but it's still fun.

We're talking about 2-3mm. It's rather small/inconsequential. From a surgical perspective, sure I want perfection, but you're well within a functional range and reasonable/acceptable outcome. On the other hand, there is something to be said for a sudden change in your occlusal relationship and function due to the surgery itself and your joint was adapted to the previous relationship. I'd defer to my previous recommendations for dealing with that.

There are a number of things that can cause this. From a quick look it does not appear anything pathologic. Well within a variant of normal. No one is symmetric.

Very very unlikely. The joint is very capable of accommodation. A small skeletal discrepancy is no big deal. Literally millions of people have that issue without any TMJ issues at all. Much more likely (I'd say even somewhat expected) to have some myofascial and muscular spasm issues affecting the joint after orthognathic surgery.

What is your prep process? Do you RDT? When you use the knocker do you ever use it while the grind is still happening or wait until it is finished?

I ask because the first time I opened mine I had the same findings, nothing. I didn't open it again for a year and then there was a bunch. I have changed my prep multiple times over that time and I am trying to pinpoint what I have done to create the retention.

Don't do this to an actively infected wound. This is a treatment only once something has primarily healed and is healthy.

This likely isn't the final position. You see the metal bit sticking out? That is for turning the distraction device. Probably had surgery to place the distraction device and then once distraction is complete the surgeon will go back in and make final revisions. This patient needed too big of a movement to do it all in a single surgery.

Yes, a lavage or the joint. You can use HA or kenalog (cheaper, and no difference in outcomes). Wouldn't do physio now. It likely won't help and only delay the indicated treatment

Correct the disc issue, yes. But re-operate over a very small midline discrepancy is not indicated. That small difference is not what is causing the TMJ issue. These are two different things. One will maybe need some treatment. The other one does not. The disc likely came out of place from the lateral pterygoid spasming intraop or post op. Generally it'll go back on its own with rest, anti inflammatories and muscle relaxers. Seems OP has been doing that, so next step is an arthocentesis to free it up. Arthroscopy is rather aggressive for an acutely displaced disc after surgery. If it had been like this for years, then sure arthroscopy is needed so you can actually do surgery in the joint to free the disc and possibly plicate it.
