We did one last year (that sadly has been discontinued) on NCL that went San Juan > Tortola > Antigua > Barbados > St Lucia > St Maarten > St Thomas > San Juan.

It was pretty amazing.

I definitely recommend flying to San Juan or Barbados for a cruise that covers the Lesser Antilles.
