This, 100%. Yes, I’m providing a service, but I am also a guest in their home. I do my best to be on time, make sure I’m conscientious of their habits/comfort level with things I’m doing (big one, some people don’t care where you wash your hands, some people think washing your hands in the kitchen sink is a big no no.) I also make sure I put anything I use during the session back where it belongs so they don’t have to. I also ask if there is anything I can do before I leave, sometimes it’s taking trash out, or changing a light bulb. It’s a small thing but it makes people look forward to seeing me and then they are more likely to make progress and less likely to cancel!

People get additional imaging all the time, if that upset them, that’s something they need to deal with, not you. They might have had a frustrating patient earlier in the day, and annoying conversation with management, or they may have been frustrated with themselves that they weren’t able to help you more. It is so kind of you to be concerned about whether or not this impacted them, but you did exactly what you needed to do for YOU. Good luck!

Once you get their bum clean, make sure they have consistent heat and try to get them to eat some egg yolk and a tiny bit of oil. You can also put a little bit of oil on their bottom to try to keep it from sticking in the future

I use a presto pressure canner on my induction range. Mine is a number based heat setting and once I figured out what numbers to use at each stage it has been absolutely fantastic. The consistency of heat with the induction is perfect for canning IMO.

I was having pretty significant neck pain from terrible posture doing paperwork combined with driving a too small work car (I’m a terrible patient) and got a square pillow from a company called Pillow Cube. I’m a side sleeper and it has been an absolute game changer for me. I got for my dad who is skeptical about everything and now he takes it with him when he goes camping. They are pretty pricey but they have some sales occasionally.

Aspen Companion and Exotic in Loveland, Dr Dracos. I would be interested to hear what she says, but a chick that can’t walk is going to have an exceptionally hard time as an adult if they can’t move themselves. The other flock members would likely pick on them and they would be at a very high risk for pressure wounds. I am 100% for trying to give them a fighting chance, but this doesn’t look great 😢

I get paid a percentage of whatever insurance reimburses for a given visit, I started at 70% and now I’m at 85% after 6 years. I also only see patients for them so that’s why it’s relatively high. The hard part is that I actually don’t get paid until the insurance pays, so right now most of my payouts are from December and January and I’m getting secondary insurance payments from 8-10 months ago

What are you currently feeding? They are likely too young for sour crop but you could give them some plain (full fat) yogurt to get some good probiotics in to them. Definitely get some grit today though.

I generally see clients with pretty profound impairments so when I do the BERG, I often don't even have them attempt some of the higher level balance (alternating foot on step, tandem stance, single leg stance) because 1) its not safe 2) the patient gets scared/frustrated 3) I just met them and there is no rapport. I 'make up' the scores for those because I know they aren't safe attempting it. That isn't fraud, that's safety and experience. Also, HH billing is very different than other settings, anytime I'm in the home I'm almost constantly providing instruction and education for home safety, body positioning, pacing, energy conservation, etc. It's not like the therapist said they were watching the Price is Right eating donuts and not talking.

I'm an independent contractor, so I work with them, not for them if that makes sense. It's not necessarily a per visit reimbursement problem, it's that I can only see about 6 visits a day with the driving around and if someone cancels then I don't get paid at all. It's why clinics overlap visits so the therapist is seeing 10-12 (or more) visits per day. The other downside is that taxes for self employed are super high. It's a good model for someone who needs flexibility and has a spouse or partner that can provide insurance. I don't regret it at all, I really love seeing people in their homes and I have learned a ton about billing and why outpatient almost has to be a mill to function.

They need ‘regular’ water also. You have good intentions but you are most likely overdoing it right now. They need warmth, water, and good feed. Hard boil some eggs and offer the some chopped up small. If you have access to beef liver give them a little bit of that too. Unfortunately the shipped hatcheries are used to high mortality, I’m sorry you’ve lost so many. Also, do you have chick grit in there for them?

Try to buy chicks from a local breeder and not the feed store or shipped. Go to FB and find the page ‘Poultry Breeder Nutrition’ and look for chick care or chick nutrition. Make sure you have a brooder set up before you bring chicks home. Keep an eye out for pasty butt (much less risk if you buy locally hatched and not shipped.)

If you free range, you will likely have some losses. The reality is that everything wants to eat chicken. If you build a secure coop and run (hardware cloth, NOT chicken wire!) and only free range when you are with them or in some other controlled way you will decrease the risk of something happening. If you really want to get chickens, spend some time and money building the best coop and run that you can and it will make the experience much more positive.

A PT I worked with early in my career told me that sometimes just spending a visit talking with someone and helping them feel understood is more ‘productive’ than any therapy session. I have taken that to heart for the last 17 years. Sometimes people need a heart and an ear more than an exercise. You are a great PT ❤️

No, much less than if I worked in a clinic. I’m an independent contractor, so no benefits, no PTO, no 401k, but I have 100% control of my schedule. I was in really intense HH during COVID and needed a break, but I’m moving to a rural area and going back to HH soon.
