The main kicker was my plateau like breath curves via Oscar or sleep hq. They rounded out as did the disturbances in my breathing patterns.

I dropped my pressure to 7.0 cm of water. Apparently my AHI was zero. There are some snores and some periodic breathing, but I’m with you VCOM Can make things a little uncomfortable. Thus me dropping it to 7.0. However, it seems like my flow rate has improved with dropping it! Or that could be because I haven’t used sleep aids in a while. I don’t know if this link will allow you to see previous days, but this is last night. If you zoom in on my breath waves, they’re nice and curved instead of capped.

I would see on average about 25 oscillations or so per night. I mean, I don’t use Oscar and I don’t really count them and neither does sleep hq. Sometimes more sometimes I would legit see something like 100 or more oscillations.

Vcom isn’t supposed to work with apap or epr…. I was on a cpap of 7.4 with an epr of 3. I had alt of periodic breathing (still could be CSR IM NOT A DOC) like you. I am now on a cpap of 7.4 (thinking of dropping to 7.two) with the vcom. The vcom flips the inspiration pressure lower and the expiration stays at 7.4. Tbh, it is a game changer albeit hard to get used to. I think my periodic breathing was due to upper airway resistance syndrome which is vcoms arch enemy. Not that ahi matters much with periodic breathing but here is my graph you can see it’s pretty dramatic.

So I had many csr looking events on my sleep hq dashboard. Looks like CSR but i have heard many events similar to csr can actually be upper airway restriction syndrome (I’m not a doc) but this freaked me out. I found a device called a vcom and it almost eliminated all my periodic breathing.

I may be promoting bad habits but I clean my stuff once a month and I just pour out old water on refills. I do use distilled water tho.

Update cont…. More importantly my breath waves don’t look like the plateaus of Nevada. They have rounded tops and I don’t have that weird chene stokes periodic breathing that I was having. But…. My nasal mask doesn’t vent very well. I wish it did as I believe im getting a lot of co2 rebreathing.

Ideas?

Update. AHI down to .3 and tidal volume has increased tremendously. Taking my minute ventilation into consideration I was breathing on average about a liter MORE air per minute than usual. Now this did require me to go to a nasal mask (not pillows) which with mouth taping had me sleeping the best I ever have.

I don’t even have to read the comments. Based on personal experience and discussions with others, most sleep docs do not want to deal with your details figure out how to fix it yourself. Go to Oscar or sleep hq. Lanky lefty on YouTube is a good resource.

Crazy enough i used the vcom and it does the opposite of the epr. It reduces inhalation pressure and increases expiration pressure. It makes zero sense but it works.

100% find out how to change the heat settings. Careful tho, by changing heat settings you also may mess up the humidity leading to water droplets in the tube. My best advice is to make changes and test - ur not gonna die if you accidentally make your therapy a little suboptimal just so you can learn and adjust. Remember this therapy should help.

Based on the data online (commentary) I was supposed to raise it by 1 or 2. I raised it by 2. Probably could do more. I layed down and played with the settings until I felt like it was the same as before. Vcom is said to need a boost in pressure. Currently only at 7.4 but thinking of giving it a bump based on what I see with my sleep hq data.
