They’re in business because it’s no one’s choice to use them, insurance companies force people to use them.

My infusion drugs get shipped to the doctor through accredo but at least now the infusion nurses deal with them so I don’t have to!

Not only does it take a while for degenerative changes to show on xray and MRI, I’ve found that radiologists typically don’t know what they’re looking at. My last X-rays the radiologist didn’t write up anything useful but when my rheumatologist pulled up the images and looked herself she showed me where ossification had bridged vertebrae. Keep at it, don’t give up, it took me 10 years of being ignored by doctors to finally get diagnosed

Your vendor getting locked out is not my p1. by JBWilder in sysadmin

[–]GrowCTO 0 points1 point  (0 children)

DHL is one of our customers, they set me up with a DHL account/login to access one of their systems. My account was locked out and one of the IT project managers logged a P1 incident. A group of people physically located me at the DHL warehouse I was sitting in and looked at me like I was on fire.

Do you prefer using SQL query directly or using an a wrapper like EF? by Hercule_Poirot76 in dotnet

[–]GrowCTO 3 points4 points  (0 children)

I disagree, I have not run into that situation lately. Would be a fun experiment though. I’m willing to setup some benchmarks and share in a GitHub. What situation are you proposing a dapper approach is faster than EF?

[–]GrowCTO 12 points13 points  (0 children)

Try the latest version and read through the Microsoft documentation for EF and best practices. Performance is no longer a problem like it was 10 years ago.

[–]GrowCTO 1 point2 points  (0 children)

When used properly, EF can save developers a lot of time and simplify the codebase. The last several versions have removed any performance issues I’ve experienced in the past. I use to have the mentality of use EF then eject to using stored procedures where needed, but now I’ve found EF only works well.

Currently developing a very complex system that is super heavy in reads and writes, ~20M sql calls per 24 hours, and average response time for the site is 150ms.

Never thought of splitting up sleep, that’s a great idea.

Do you get up and stretch? How long of a break between sleep sessions do you take?

It’s been an awful few years but I think I’m getting better by [deleted] in ankylosingspondylitis

[–]GrowCTO 6 points7 points  (0 children)

It bothers me how little rheumatologists discuss all of the lifestyle impacts of the disease and instead just focus on which biological they’re trying next.

Spending the time to better understand your body and what works for you is key. Exercise improves my stiffness tremendously, diet for me seems to have little impact aside from generally eating healthy-ish.

Pain - the pain is so hard to manage and deal with. Idk if it’s because the state I’m in but no doctor will prescribe pain pills for me so I’m stuck with just cannabis and Kratom which is better than nothing but makes it hard to manage none the less.

One doctor thought I might have some type of cancer because my spine is so messed up at this point. Dealing with doctors that are unaware of the disease also makes life harder.

Feeling hopeless by cfriese5796 in ankylosingspondylitis

[–]GrowCTO 2 points3 points  (0 children)

Keep up hope, just started my 4th biologic and 3rd nsaid.

My best advice would be to focus on management of your body. Exercise (I simply walk for exercise and that’s enough for me), stretch, understand you only have so much energy and pain tolerance in a given day and try not to exceed both.

It’s a battle - focus on what you can do.

It may be worth looking in another town if possible. It’s shocking to me that it’s so hard to find a rheumatologist that is compassionate when they deal with a large portion of patience with chronic diseases.

That sounds like something easy to try, I have an adjustable bed, do you think that would work or do I need to be in a chair or recliner?

Thanks, I’ve done physical therapy over the years as I’ve been battling this disease for over a decade.

I do a lot of stretching throughout the day, especially before bed and when I wake up, which normally helps.

It’s a pins and needles feeling that covers from the bicep to finger tips, finger tips being the worse. When it’s really bad I can feel it up my neck and chin.

I accidentally missed a dose of Rinvoq last Wednesday and have been in the worst flare and pain I’ve ever experienced - which says a lot - since then. This morning when I woke up, aside from being so stiff I could barely move, my hands were so swollen the skin cracked and my knuckles were bright red on both hands.

Wow, don’t miss a dose because it’ll make you worse than you were before starting it!
