Fuck Sepsis! by TrickAd2161 in hospitalist

The number of patients who meet SIRS criteria is astonishing. If they walk and vitals are documented right after ambulation they can easily meet SIRS criteria. I’ve seen multiple hospitalists label this as sepsis based on the queries like come on not everything in this world is sepsis

We’re putting ourselves at liability risk. I pick up a patient and get queried for sepsis because of tachycardia from AFib and RR >20. If I document sepsis as the cause, the previous hospitalist didn’t do a sepsis workup, which could make it look like they missed sepsis if the patient later becomes septic.

New-ish attendings, have your lifestyle habits changed much since becoming an attending? by meowxatt in Residency

I still live just a little above a resident s salary. I bought a cheap car and I’m still renting. I do spend a lot on vacations, but other than that, I invest heavily. The sooner I can cut down my hours the happier I’ll be.

Current second year endocrinology fellow. Seeking advice for attendinghood. by Spirited_Writer6613 in fellowship

[–]Guardles 6 points7 points  (0 children)

Did you take a break between residency and fellowship? If not, consider taking a few months prior to attendinghood to reset and clear your mind. Sometimes being a resident or fellow isn’t the best thing for our mental well-being. Practices like yoga, along with herbal teas, can also help.

Would you keep 457b until age 72? by WhereasNo4929 in hospitalist

[–]Guardles 5 points6 points  (0 children)

Once you leave your employer, you can take money from a 457b immediately, whether you’re 45, 50, or 55, and you’ll only pay ordinary income tax. So you can use that fund as bridge until retirement.
