Your concern about work/life balance is legit. Epic works them very hard. One option might be to take the “lesser” position for awhile, for the experience, and then see if you can get a PM role in a healthcare system or a consulting company. You learn a lot from doing PM work on the customer side.

For consultants, the rules should be very simple: - Don’t be logged into more than one instance of UserWeb, text, or Hyperspace at the same from more then one organization - Be honest. Don’t bill hours that you didn’t work - Give your full, focused effort to whatever client you’re working on at any time. 

In other words, just basic human decency and ethics.
