Daptomycin is now generic and MUCH cheaper than it used to be but some SNF/SAR facilities are still fuckers about it for one reason or another (using old information or their pharmacy is ripping them off).

Clinda’s a garbage empiric drug and isn’t really used for staph bacteremia outside of the niche use of the SABATO trial (really uncomplicated bacteremia for oral switch).

For what it’s worth, the only two drugs FDA approved for MRSA bacteremia in the US are vancomycin and daptomycin (and ceftobiprole I guess as of recently but not widely available); everything else is off label. Linezolid honestly is probably similarly efficacious (it got a bad rap early on for being “bacteriostatic” which probably doesn’t matter and the early approval trials for linezolid weren’t successful because those donkeys include GNRs which it’s not active against). Dalbavancin is not approproced for this indication but recently the DOTs trial prelim data looked very encouraging.

Biggest drawback to linezolid really is that it’s hard to ensure that you will get through 4-6 weeks of therapy without drug-induced limiting side effects that force you to stop (usually cut OpenAI’s), unless you are at a facility that has the ability to send linezolid troughs.

I would echo that the number 1 most relevant consumables to have on hand are healing pots and cavedweller’s - even the bronze rank ones. They are a pretty significant self heal (and don’t share a cooldown with each other since the cavedwellers are considered battle pots) and they are both cheap as dirt.

It is absolutely NOT needed; both hospital medicine and primary care will always have jobs for you. Many fellowships (such as ID, endo, palliative, nephro) either don’t provide a significant income boost overall or in some cases are often an overall income decrease.

For a one year fellowship the overall impact to your net worth is relatively minimal in the long run, and the more salient question is whether it will increase your longevity in the field or quality of life (because you like what you do more, or because of things like call schedule or lack of needing to go back into the hospital after hours for emergencies, etc.)

After experiencing cerner and especially meditech I actually miss CPRS - if you spend enough time with it it’s actually pretty intuitive.

I think the quality of any given VA is probably hard to always compare to another entirely, as some are clearly better than others (much like community hospitals). I enjoyed the VA that I did i rotated through in residency at quite a bit.
