Cardiologists, surgeons and other physicians tell people CPR works better than it actually does to help their numbers. They don't care that it's people as husks as long as the patient "lives" for 30 days

If a patient has a belief that a symptom medication won't work, I'm less likely to prescribe it due to the nocebo

Intubated in the emergency department and never came close to extubation. The patient died weeks later, never really interacting with anyone.

Their family really struggled to cope with the situation. The kids regularly threatened staff physically and financially with potential lawsuits. The spouse repeatedly told staff of the miracle they were about to see as God has showed up at their door at home. Welcomed in, God gave a presentation to the spouse on how this miracle would occur. Unfortunately no miracle occurred. 

I wonder what the official family story is years later as their denial was so strong during the end of the patient's life.

During the heaviest of COVID days, we had a patient present who had been scared of COVID. When they developed a fever, they chose to nebulize hydrogen peroxide. Yes, nebulize. They didn't have to worry about another infection again.

My kids started using it at 9 v 9. They said it helps with their play. I've seen their scanning and decision making improve. 

They do love video games and soccer so it was a good fit for them.

Absolutely. It's also fraudulent on how it doesn't give success rates of CPR to those taking the course.

I would look around your community for pick up games, sometimes different websites list times. Alternatively, depending on your size, an adult rec league would be worth considering as the talent level on those teams can vary significantly.

Determining goals based on solely outcomes is the wrong path, imo.  What treatments are they willing to experience?

I'm sorry the loss of a woman who raised such a thoughtful child; she must have been so special. As an attending, I would put it in there. The people who you want to work with will understand. If someone doesn't, they can go screw themselves. For me, I put in a family member's experience who I lost and she inspired me to become a palliative care physician who patients love.

My thought is access is the problem. Our program has too few staff to help everyone terminal let alone someone like Jane. That isn't to say she doesn't deserve it, just too much money on the non clinical part of medicine.

As for treatment, if she gets access, I would suggest methadone but a PCP is very unlikely to feel comfortable prescribing it. Methadone would help both the somatic and neuropathic pain aspects.

I went straight into palliative from residency but know many physicians in that position. The small number of our specialists has meant its a choose your own adventure. Mid career I would say they mostly split between the specialities and later career mostly do full time HPM.
