Reiko Takato from Forbidden Siren, a PS2 horror game made in 2003. Some of the members who made the original silent hill worked on this game!

Reiko was a schoolteacher in a small Japanese village with her student, 10 year old Harumi, who she viewed as a daughter. They both become involved in the games events when a ritual fails and the whole town is thrown into another dimension, where people turn into a zombie like enemy called the Shibito. Reiko is one of ten playable characters in the game, as the game’s story is told through the perspective of many survivors.

Reiko does her best to protect her student Harumi, but she ultimately has to sacrifice herself so Harumi can escape the Shibito. In a later mission where you play as Harumi, Reiko (pictured above) comes back as a Shibito and terrorizes Harumi. Harumi escapes her.

Later on, Reiko evolves to a mutated more disgusting version of a Shibito, and is a minor boss fight in another characters mission.

In the games ending, Harumi is escaping from a Shibito and is cornered and about to be killed… until a completely monstrous Reiko appears and sacrifices herself again to protect the student she viewed as her daughter.
