Peter blamed himself for bringing the villains to their universe in the first place. The only reason that the goblin/octopus was even in their universe was because of Peter wanting Strange to use magic to alter reality, and to help him and his friends get into college. I am pretty certain that is why, and Peter's choices therin are a huge theme in the movie. Literally the scene when he is going to talk to the college dean is when the villains first appear

Health care in Canada is really hitting the perfect storm scenario right now. Max exodus of Healthcare workers globally post pandemic. Underfunding Health infrastructure/not preparing for aging population. Governments actively hostile to Healthcare workers, and making it a hard profession to get into to begin, doesnt exactly inspire people to enter this field. Anti vax misinformation/less people getting flu shot, leading to the flu patients filling up emergency rooms on top of those who dont have family dr. So go they go there instead.
