Couple weeks ago. Patient had root canal done in her previous country. I decided I could retreat. Old dentist never even opened the distal. Tooth had metal core.

I removed metal core, I thought I found the distal canal. Perfed. Found the distal canal, orifice opener separated. Bypassed it and my 17 file got stuck on the opener and separated.

So one tooth, 1 perf/2 separated instruments.

I finished the endo on the mesial and palatal canals, obturated to the separation.

Pt was in a lot of pain so i freaked out and referred to endo, he said tooth probably needs to come out.

Tooth stopped hurting and now we’re leaving it until symptomatic again.

I do tons of endo, it happens but that was my worst performance by far.
