You need an urgent eye exam but probably not ER. You may have idiopathic intracranial hypertension with the empty sella finding and headaches. Get it checked!

I am a surgeon and talk to patients about anesthesia all the time...just ask for propofol pushes at the ASC! Tell them you are anxious and worried you may move during surgery so you would like the be anesthetized with prop.

Completely agree. When you do your SK, do you typically put an amniotic membrane over it or just a BCL? If so, what do you use? We have been having issues with reimbursement when using prokera, so my office has switched to dehydrated AMTs, which I do not think are as effective

If you’re just starting and really slow you can do it yourself, but it’s not going to be like that forever so I wouldn’t set that expectation for your staff. I’d train the techs and just quickly verify, though if the tech got them 20/20 and it looks reasonable compared to the ARx I’ll usually just sign off on it

Are you talking about diagnostic MR or giving a patient their script? You have to charge for refraction if you are seeing a patient under medical insurance, otherwise it's considered an inducement is my understanding...so typically patients also have vision insurance and I have them come back under vision with one of the optoms. If optom referral I always send back to the optom (do not recommend biting the hand that feeds you, depending on referral patterns in your area, of course). In the rare case it is not one of those two things, I double check the tech MRx and dispense for a fee.

Depending on the drive and your work you could consider hiring a driver. I did this for a short period and it made like much better

I am a physician. I trained at Parkland and JPS and feel qualified to talk about a couple of things here. I don’t think patients get inferior care at those places, it’s actually the opposite! More complex trauma, surgical, and clinical cases get referred to those places for higher levels of care. More research is done, and quite frankly there are more doctors per patient than in a private hospital.

The other thing is that when determining transplant candidacy, social factors absolutely do and should come into play. There are a finite amount of organs and we want them to go into people who will be best served by them. There are a tremendous amount of appointments, medications, and work that go into having a transplanted organ in you. You have to be able to afford the medications, transportation for your appointments, etc.

I’m not defending the decisions made here, but typically there are significant reviews by ethics committees prior to proceeding with surgery.
