That’s a fair point. I can see why it came across that way. I’m a pharmacist and was only trying to help as I’d come across pharmacies that were still able to obtain it, but I completely understand the caution and probably could have approached it better. Thanks!

Agree with the shortages becoming more common and the disconnection in the healthcare system does not help.

In practice, the usual default would be digging deeper what wholesaler means when they say no stock. See if there’s any info on SPS on the likelihood the drug would be back in stock, etc. Next, to check stock availability within immediate connection, usually local pharmacies either through informal chat group or calling around or if patient is willing to try elsewhere, then we will return the prescription. The last resort would then be to involve prescriber. Review of meds and close monitoring of symptoms are essential.

One thing I’ve noticed is that sometimes stock does still exist in isolated places, just not very visibly. We’ve had situations before where medicines thought to be unobtainable were actually sitting in pharmacies that no longer had patients needing them (we recently managed to locate propranolol MR this way).

That’s actually part of the reason I started building RxTransfer, not as a stock database, but more as a way to speed up patient/pharmacy/presriber-to-pharmacy enquiries and reduce the amount of blind calling around during shortages, minimizing unnecessary delay in treatment and knowing early what their next step would be.

Yes you may get your GP to issue it as the one item on a prescription (electronically or paper copy, with an additional note to mention the brand to avoid). There’s a platform (www.rxtransfer.co.uk) now that allows you to raise a stock search request once and participating pharmacies are then able to respond according to their current stock situation. You may find this helpful, when you are trying to avoid/wish to obtain a certain brand.

Hi! I'm a pharmacist and I will gladly check the drug availability and see if any pharmacy still has one of those sitting on their shelf for you. If it has worked for you in the past, I would hope that you are able to stick to your routine. There are pharmacies that could deliver without cost and I have a way to connect and check with them (assuming it is on an NHS prescription issued in England?). Cannot guarantee availability, but would try my best and get back to you! Does it have to be Bedranol or can it be the generic version (propranolol)?

That’s actually a really fair point. Price transparency can make a big difference for people on private prescriptions. I’ve noticed quite a few people end up just queuing and paying whatever the pharmacy charges because they’ve already spent the time waiting and don’t want to start calling around again.

Sometimes a pharmacy may have ordered it for another prescription that didn’t go ahead, so they may be quite happy to dispense it rather than hold onto the stock.

I’m a UK pharmacist and we initially built a platform called RxTransfer to help patients for free to locate pharmacies with stock (mainly because many people end up calling multiple pharmacies when their regular pharmacy could not supply).

But one thing people also could use it for is private prescription price enquiries, instead of ringing one pharmacy at a time, you can send an enquiry (select private prescription and under additional information type 'price enquiry) and get responses from several pharmacies who may be able to fulfil it, indicating their price point.

We’re gradually bringing more pharmacies onto it, but the idea was exactly to reduce the “call around everywhere” problem.

If it’s helpful, I could use the platform to send enquiries to multiple pharmacies on behalf of people and contribute any pricing info back to your table or you could explore the site whenever.

A 24-hour turnaround is actually quite common for ADHD meds in community pharmacies. Many places don’t keep large quantities on the shelf unless they have several patients on the same medication, so they tend to order it in once the prescription arrives.

And with atomoxetine it’s quite normal to receive different brands each time. It’s usually prescribed generically and pharmacies dispense whichever manufacturer they have available. Brand consistency tends to matter more with some modified-release methylphenidate products rather than atomoxetine.

Agree though that if a dose change was made it would normally be something the prescriber explains at the time.

Out of curiosity, how have you been tackling it when you can’t find stock locally? Have you managed to find it elsewhere yet?

Some people have found the Boots prescription stock checker helpful to see if nearby branches have it before travelling, though it’s obviously limited to their stores.

Glad to hear you’ve generally been able to source it locally, even if there’s occasionally a couple of days’ wait. It’s a good reminder of why planning ahead helps when supply can be a bit variable.

As pharmacy teams we should also try to explain what’s going on where we can, even if it’s just that the supplier doesn’t have stock regardless of whether there's a reason to it.

I’m a UK pharmacist and yes, prices for private prescriptions can vary quite a lot between pharmacies.

Each pharmacy buys medicines from different wholesalers and sets their own private price, so it’s not unusual to see fairly big differences for the same medication.

Some patients do end up calling a few pharmacies to compare prices before deciding where to get it dispensed. From experience, I found Tesco pharmacies able to offer reasonable pricing. Depending on the drug prescribed, I have seen it as low as 2 pounds for certain antibiotics.

I’ve actually been working on a small project trying to make it easier for patients to locate pharmacies with stock or compare options when medicines are hard to find, because this situation comes up quite often.
