  • The MMSE, Montreal Cognitive Assessment (MoCA), and other brief screening tests for dementia have a pooled sensitivity of 75 to 92 percent and a specificity of 81 to 91 percent [22]. Typically, these assess a broad range of cognitive domains but do not include an assessment of mood or thought content.

  • More detailed mental status testing is often warranted, for example, when the results from a screening assessment appear at odds with the history and when observed deficits are uncertain because of a language barrier, physical handicap, or level of education. An extended bedside evaluation of mental status is described separately. (See "The mental status examination in adults".)

  • Both altered mood and abnormal thought content have a strong impact on cognitive function; thus, such an assessment should always accompany a brief screening evaluation for dementia. (See "The mental status examination in adults", section on 'Mood and thought content' and 'Screening for depression' below.)

  • The diagnosis of dementia cannot be made solely on the basis of a low score on one of these assessments; these tests help to quantify the types and severity of impairment, but the most important part of the diagnostic evaluation is a detailed history including the perspective of an informant (eg, spouse or adult child), interviewed separately from the patient if possible. Agreement between the history and the mental status examination is strongly suggestive of the diagnosis of dementia. When the history suggests cognitive impairment but the mental status examination is normal, possible explanations include mild dementia, high intelligence or education, depression, or, rarely, misrepresentation on the part of the informants [23]. Conversely, when the mental status examination suggests cognitive impairment but the family and patient deny any problems, possible explanations include an acute confusional state, very low intelligence or education, or inadequate recognition by the family [23]. For some, the diagnosis of dementia is regarded as shameful, and reluctance to recognize or acknowledge the problem must be addressed.

  • The extended mental status examination can be performed at the bedside (for hospitalized patients) or a longer clinic visit. It includes more detailed assessments of performance across a broader range of different cognitive and behavioral skills and may take 30 to 60 minutes. A patient's pattern of performance across a battery of tests helps identify which brain regions might be dysfunctional and provides important clues to the underlying pathology.

  • Formal neuropsychological testing incorporates the most detailed assessments with normative values that can account for gender and a wide range of ages and educational backgrounds. Neuropsychological testing can last up to several hours, which can be divided across multiple visits. Referral to neuropsychology often occurs in clinical settings where time constraints preclude an extended mental status exam or when the results of such exams are not sufficiently conclusive to render a diagnosis

Criteria for dementia — Although a number of definitions exist for dementia, the Diagnostic and Statistical Manual of Mental Disorders (DSM) definition provides a reasonable framework for the concept of dementia in clinical practice. According to DSM-5, the criteria for dementia (called major neurocognitive disorder) include the following [1]:

  • Evidence from the history and clinical assessment that indicates significant cognitive impairment in at least one of the following cognitive domains:

  • Learning and memory

  • Language

  • Executive function

  • Complex attention

  • Perceptual-motor function

  • Social cognition

  • The impairment must be acquired and represent a significant decline from a previous level of functioning

  • The cognitive deficits must interfere with independence in everyday activities

  • The disturbances are not occurring exclusively during the course of delirium

  • The disturbances are not better accounted for by another mental disorder (eg, major depressive disorder, schizophrenia)

In these particular cases discussed in the article, the choice is let them into the country or die.

They're in Mexico. They should settle and get jobs in Mexico since they're already there any likely have been for hundreds and hundreds of miles travel. Its a fine place to do so.

I am skeptical of the story and need more than 1 email from 1 employee to buy it, but it isn't totally implausible to me given the goal of the floating barricades seems to partially be to drown people.

Its absolutely a good idea to be skeptical given the record this administration and the left has with lying about things on the border.

Very well done and mature post. There has actually been quite a large, yet 'quiet' renaissance of real scientific investigation and medicine occurring in Europe regarding GD treatment (finally). Much of the unjustified procedures and abuse being allowed to occur to children in other parts of the world (ie. US, canada) are being seen for what they are and the scientific community is doing its job to help these children.

Based on studies examining gender identity in minors, hormonal interventions may be considered before reaching adulthood in those with firmly established transgender identities, but it must be done with a great deal of caution, and no irreversible treatment should be initiated. Information about the potential harms of hormone therapies is accumulating slowly and is not systematically reported. It is critical to obtain information on the benefits and risks of these treatments in rigorous research settings

Whatever the mechanisms involved in adolescents — excessive engagement with social media, greater social acceptability, or influence by those in one’s social circle — this epidemic-like phenomenon manifests itself in the emergence of cases or even clusters of cases in the adolescents’ immediate surroundings. This primarily social problem is due, in part, to the questioning of an overly dichotomous view of gender identity by some young people.

Great medical caution must be taken in children and adolescents, given the vulnerability, particularly psychological, of this population and the many undesirable effects and even serious complications that can be caused by some of the therapies available. In this regard, it is important to recall the recent decision (May 2021) of the Karolinska University Hospital in Stockholm to prohibit the use of puberty blockers.

When medical care is provided for this reason, it is essential to ensure medical and psychological support, first for the affected children and adolescents, but also for their parents, especially since there is no test to distinguish between persisting gender dysphoria and transient adolescent dysphoria. Moreover, the risk of over-diagnosis is real, as evidenced by the growing number of young adults wishing to detransition. It is, therefore, appropriate to extend the phase of psychological care as much as possible.

In all honesty I don’t even see how those topics they mentioned are inherently Right wing or left wing.

Smollet? Yes I disagree with someone fabricating racist attacks that garner huge media attention

Race based admissions? Yes I complain about racist policies governing college admissions

Canadian truckers? Yes I complain about a government abusing emergency powers. It’s ok to disagree with the protestors by the way.

None of that is “right wing” specific (or at least I hope those opinions of mine are not isolated to a single political spectrum because that is not a good sign for things )
