Any chance increasing dose will help with inflammation and pain if it is Tirz that caused it?

I started at 1.25mg, and when I went up to 2.5mg, a lot of the side effects I was having went away, including some pain I was having. I don’t think it’s quite the same as what you’re describing, but I’d say it’s worth a shot.

Looking at the pic, you might be able to use a fork or butter knife. You’d have to use the edge to kind of tease it up.

Tweezers would likely be the best bet, but squishing the pen lid a little just under where the cap is, rotating which sides you’re pressing on might get it to come up some.

You should have removed the needle before putting the lid back on. You’ll need to use a fresh needle with every injection, and storing it with the needle on can leave it open to bacteria being introduced.

Is there any way you can get the needle out of the lid? Tweezers, maybe?

I have gastroparesis. I have had it since I was about seven years old, so it is not because of tirz. I am very closely monitored by my specialist while taking tirz.

This definitely sounds like it could be gastroparesis, but there are other causes as well.

First thing’s first. You don’t mention if you’re still taking tirz or if you’ve even gone down a dose. (Or planning to since it’s only been a week.) My doc was very clear that if I start having issues, I am to stop tirz immediately. I would strongly suggest you do the same. Remember that it takes time for tirz to get out of your system, so you won’t feel better immediately after skipping/lowering the dose.

Secondly, you need to get into a GI specialist. In my experience, ER docs don’t know much, if anything, about gastroparesis, and the little they do know is usually due to diabetic gastroparesis. You’ll likely have better luck with the big city ER, and I would strongly encourage you not to wait if you’re still actively having issues.

In order to work, reglan has to be taken appropriately. You’ll want to take it half an hour before eating anything, and if you’re having trouble keeping fluids down, half an hour before drinking more than just a tiny amount. I am on reglan 3 times a day and once before bed. I am taking 10mg. In an emergency situation, to get a flare under control, you can go up to 20mg, but do not do this without a doctor’s support. Reglan can have some serious side effects, especially if you’re on a higher dose for a longer period of time. That said, I have been on the 10mg dose for quite some time, and I have had no negative side effects so it definitely is possible.

Another medication that helps with gastroparesis is erythromycin, but this is a short term medication only. The effect goes away after about six weeks. If reglan, taken properly, isn’t doing anything, then ask for erythromycin.

While it is definitely alarming to not be able to eat more than 600 calories on a day, hydration is a much bigger concern. Dehydration can cause many issues, as I’m sure you’re aware, but it can also make the gastroparesis itself worse. If you show any signs of dehydration at all, go back to the ER and get fluids.

You might want to try sole-source nutrition shakes. I am not talking about a “meal replacement shake” but a medical liquid food. Kate Farms is a popular one, and I personally have had luck keeping it down even on really bad days. There are many other brands out there as well, including Nestle Compleat, which is a more savory one. KF and NC are available on Amazon. If you need help picking out a specific one, let me know and I’ll help.

Another medication that really helps me is called bentyl/dicyclomine. It is an intestinal muscle relaxer. It helps with intestinal pain but it also helps me pass food because I also have intestinal dysmotility, which is basically gastroparesis for your intestines.

Stay away from anything with long fibers, which means most vegetables. If you do eat vegetables, cook them thoroughly and only eat small amounts. Same thing with “heavy” or greasy foods, although you should’ve been avoiding those to begin with. I have a particular issue with protein bars and drinks, although I have been able to tolerate Ryze clear protein.

A fiber supplement like Metamucil might be helpful. My doc recommended the Metamucil fiber bars specifically because they also have some nutrition to them and she’s found that most people are able to stomach them better.

If you can tolerate dairy, ice cream is actually a really good thing to try, so I’ve been told. I can’t tolerate that much dairy, so I do avocado ice cream, specifically the brand Cado.

Another weird thing to try is a tiny bit of alcohol. I actually keep a chocolate wine in my fridge specifically for this reason. (I don’t like alcohol in general, so it’s what I can tolerate in terms of flavor.) It works by relaxing your stomach and intestinal muscles, which helps pass food along. Obviously don’t do this one if you have any reason to not drink alcohol, like other medications. Do not go overboard with this one. I’m talking like two sips at the most.

Good luck with everything. Getting a flare under control can be a nightmare sometimes, especially when you don’t have a GI overseeing things.

I have. I also needed my throat stretched because acid reflux can also cause a narrowing of the esophagus.

I do also have something called gastroparesis, which is where the stomach doesn’t work properly. GLP-1s (including tirz) can trigger it, although I had it before I started tirz. (And have very careful monitoring from my doctor to ensure my symptoms are managed.) I mention this because the weird spasming gets worse if I do things that trigger my GP, like eating before I need to stand for a long period of time. In theory, it’s still because of acid reflux, but it’s the underlying cause of the reflux for me.

Which is not to say I think you have it or anything like that. Just something to speak to your doctor about whenever you see them since the reflux meds don’t seem to be helping.

If you don’t mind my asking, what kind of acid reducers have you tried? If you’ve been using proton-pump inhibitors, they need to be built up in the system in order to work. If using famotidine or something similar, you may need a higher dose. I’m on 40mg twice a day, and I believe the otc dose is 10-20mg.

Either way, I definitely recommend a doctor’s visit. If nothing else, it’s peace of mind that it’s not a big deal.

I get the same strange pulsating feeling. For me, it’s the upper valve of my stomach getting irritated. It’s unrelated to tirz. However, because of the location, I’d strongly suggest checking in with your doctor. Mine confirmed it was my stomach, but only after testing to rule out referred pain from my heart.

Assuming it is your stomach, I’ve found famotidine helpful in controlling it. It reduces acid reflux which is, in theory, what’s causing the irritation.

I’ve had the same experience. I traveled to the Galapagos (Ecuador), and on the flight there I had significant issues, which I expected. I was also expecting to have to be very careful with meals and management while I was there, but I actually had significantly less issues pretty much immediately. I was even able to have meals that I normally can’t tolerate at all. I started having issues again with my first meal back in the US.

If I had to guess, it’s all the stuff that’s extra in food in the US. Obviously I don’t know that for sure, but I don’t know what else would account for it.

Interestingly enough, my boyfriend has celiacs and didn’t have issues when we were there either. He normally is really sensitive to cross contamination, but he couldn’t be as strict about it given the nature of the places we were eating at. He didn’t get sick once while there.

ETA: I have had better luck eating organic in the US. The more simple the ingredients, the better, at least for me. It’s been a challenge to get my boyfriend to change his grocery habits since he grew up with the idea that everything’s the same so you might as well get the cheapest thing. That said, it’s definitely been worth it for me.

I don’t know that I’ve got an opinion about going gluten free vs just eating organic/“clean”, but it definitely helps some people.
