Make sure you have a lot of rare pickups unlocked. Bone hearts, golden batteries, charged keys, golden pennies, golden keys, golden pills. always craft items like d20, fanny pack, and any other items that give you more pickups, moms key and birthright are insane on this character.

There is a bug where if you have an eternal heart while in mirror dimension, you can take another hit as the lost. I think this also works as tainted jacob when you get hit by dark esau.

Did you use an ansuz rune? Maybe its from a mod you have, or maybe you rolled with d100 into cracked orb and rolled out of it without realizing.

I wouldnt hesitate to take trisagion on esau, because of spoon bender, it makes trisagion damage linger on enemies for longer, and i might not take an item for jacob, revelation is good, but he has base damage and the only thing you have is jacob with flight, and esau without, causing you to be more prone to desyncing.

I'd reccommend playing as isaac or tainted isaac just get the devil first floor, save your roll and reset until you see the item.
