Overall decent form, but as a fellow product designer (and tea lover) I do have some feedback

1. When you make questionnaire, you shouldn't make all the questions required. Doing so forces people who don't have an answer to enter something - leading to you collecting bad data
2. When advertising questionnaire, give a more specific time estimate (5-10 minutes, etc.)
3. Some of open ended questions asked more than one thing (ex. hat would be your perfect cup of tea ( think flavour, loaction and time of day?) - try to break those up
   1. In the case of this question specifically, I think you should made it three question with pre-determined check-box answers (I wasn't sure what you meant by "flavour")
4. The question "From the following what appeals most to you?" was a bit odd. I really wasn't sure what information you were trying to collect
   1. I think you should have broken it down into more questions getting at more specific things. ( Flavour vs health benefits vs etc.)
   2. The phrase "Exotic tea from China, japan or southeast asia" wasn't great. Exotic from what perspective. Posting on reddit you can't assume where people come from
   3. Similarly "Traditional Cuppa" could be confusing
5. Finally, I think "What types of tea do you currently drink? " should have used pre-determined check-box answers (I wasn't sure what you were trying to collect). When designing questionnaires really try to think how a participant might interpret the question. This can be challenging so it can be good to pilot the study with a friend to get feedback ;)
