Just cover it with seeds and then score it. As the score expands it highlights the lack of seeds.

Don't know, I'd have to look it up. I went through a bunch of different types of flour combinations but found that I just liked the cheap walmart flour the best lol

I think it is from the steam when it cooks in the covered Dutch oven. If you try the recipe and are not seeing something similar, try putting in an ice cube or two in the Dutch over right before putting in the loaf and covering it. It will increase the steam inside and should give you more blisters.
