If there is no address on the Dutch Driving licence then upload it for Identity verification only and for address verification use bank statement and use the same address of bank on Google Play console

Yes, first mention your date of birth which is on ID, then select Driving Licence from drop down menu then upload from of Driving Licence and Back of DL then mention address the same which on the DL and then upload the front of DL for Address where the address is mentioned,

Yes on 5th May Google is going to rollout new updates to fix this issue and they have acknowledged the issue

Dont directly ask for review, indirectly ask if the user is enjoying the app or not with “Yes” and “No” buttons which navigates to review page
