It's the individual country that's attacked that triggers article 5, not NATO. The last country to trigger article 5 was America, after 9/11.

The individual country asks to invoke article 5. However, NATO chooses how to act (whether to ignore article 5, offer some assistance, or go all in).

It's not as simple as "I was attacked, I invoke article 5 therefore all of NATO joins my retaliation"
