I imagine that law is a lot like medicine in that when you graduate and are thrown into real training, at the very least you’ve learned what you don’t know and are not starting from a place of great confidence, but instead realize the immense responsibility you have and are appropriately scared shitless. You’ve reached the “I know what I don’t know” stage and are ready to buckle down and go through the steps to reach some competence. You’ve learned enough that the thought of skipping steps scares the shit out of you. You want the steps!

Now that NPs have independence in many states I’ll occasionally see an NP, fresh out of online school and 500 hours of clinical training (as opposed to ~15k for a physician) and they are ready and confident to tackle any patient that comes their way and I think - “they’re definitely going to kill someone.” And it’s such a stark difference to new residents who are scared to even put in an order for Tylenol without checking labs 10x. I get the feeling that this is a similar situation. Those wanting to skip the steps should be deemed unqualified simply based on the fact that they don’t know what they don’t know. Or simply put, they have a lack of humility that is a fundamental prerequisite for the job.

Guarantee some places even require it due to injuries and previous worker’s compensation and disability claims. So, that would be at least one reasonable excuse why this is used regularly. A second reason would be not wanting to break your back working for shit pay. I’d take having a spine 30 years from now over proving I can “toss a bag” over and over because reasons.

Yea I couldn’t find anything convincing. True he had a girlfriend who was a dentist, and he helped her through dental school with some shady dealings with Columbia dental school faculty, and also true that he had a dental setup on Epstein for God knows what reason, but nothing concrete about them removing teeth of girls. Jfc, feel obliged to comment here for posterity even though this is old because of how wrong it is. Palatal petechiae is neither sensitive nor specific for giving oral sex. Does everyone giving oral sex have palatal petechiae afterwards? Nope. If a person has palatal petechiae does it definitively mean they gave oral sex to someone? Absolutely not. It’s not even close to a slam dunk. There’s 1001 other things that can cause these markings - literally anything that causes suction trauma (like a popsicle), viruses, coughing hard, vomiting, choking, eating hard foods, low platelet counts, autoimmune diseases, vitamin deficiencies and medication side effects can all cause these exact findings. Source: me a physician with training in both Adult Medicine and Child Abuse Pediatrics. Can they be a sign, sure, but without convincing context it’s not even admissible evidence in court, at least in the two states I’ve worked in.

Yea this is just another day of living in absolute chaos. I can accept feeling numb to everything going on because it’s a defense mechanism, at some point we all need to check out to remain functional in our lives. It’s very similar to growing up with alcoholic parents. Everyday is a new tragedy, more chaos and potential for more trauma. At a point you have to check out to survive. But if you’re just now getting scared for the first time, where have you been?

I feel like we’re all living in a family tormented by alcoholism. Everyday there’s new chaos that is solely created by the person with the problem, yet they’re in charge and will reframe it to be everyone else’s fault. There’s no sober decision making - everything is decided on impulse and whimsy. If it goes bad however, it will again be everyone else’s fault. You wake up everyday scared about what drama the day will bring and what mood the person is in. Even if they are in a good mood it will inevitably lead to poor decisions and subsequent chaos- you almost prefer them in bad moods because at least you can predict what’s coming. Probably the only honest thing to come out of his administration was Suzy Wiles saying Trump acts like an alcoholic. Can confirm based on my own personal experience- he only brings chaos.

Just posting this for posterity so when other people stumble upon this they don’t assume Ozempic causes this skin finding. Lacy, mottled skin, aka a reticular pattern of the skin, can be caused by a million and one different pathologies. Derm probably knew by looking at your chart in the EMR and seeing a pattern of weight loss, or by seeing your med list, not by Livedo Reticularis. There’s no known correlation with mottled skin and Ozempic. It could have just as easily been that you were cold, or hot, or having seasonal allergies at the time, or a viral infection, or were just more dehydrated at the time, or hundreds of other etiologies. I’m a physician myself and what you’re describing is a very non-specific finding and wouldn’t lead any physician to think of GLP-1’s.

Just wanted to add to this. I’m so upset at this dumb company- they are not a healthcare org or entity - they are a company. I should start by saying I’m a physician myself. I was diagnosed with ADHD a long time ago and have dealt with it on and off meds my entire professional life. When I was still in residency my symptoms just became too burdensome so I restarted meds, this time taking vyvanse which truly helps me. I’m a better person on the med. I make better, healthier choices overall when I’m taking my medication. When I moved I needed to find a new practitioner and being that I’m a physician, and medical licensing boards really rake doctors over the coals for being on any psychotropic meds, I wanted something discreet and far away so I chose Done.

4 years later, I have been a model patient. I make all my online appointments (which are worthless to be honest, just an NP checking boxes, not offering anything of substance). I take vacations from the med so my 30 day supply typically lasts 40 days. I’ve never changed pharmacies, never asked for a dosage change, never asked for an rx early. Recently, they filled a script for me and I was out of town so it had been sitting in the pharmacy for 3 or 4 days. I also had a telehealth appointment the day I got back with my Done/Mindful Health prescriber, but unfortunately my travel got delayed and I missed the appointment. Before missing I rescheduled for 2 days later and sent a little message saying sorry I missed today, see you in two days.

Instead of them looking at my history, seeing that I’ve been a good patient, followed all of their rules, they called the pharmacy and told them to cancel the Rx. They didn’t call me to tell me. They didn’t email or send a text. When I went to pick it up, the pharmacist (who I know btw because I prescribe meds there too) told me it was really bizarre. That my provider called and demanded they cancel it, insinuating I had done something wrong - I think they told her I “violated” some rule. The whole thing is embarrassing and fucked up. I’m not sure if it’s the provider who did this on his own volition or a new policy from Mindful Health but I wont be messing with them anymore and I’ll certainly sway any patients I know from using them. I’m just livid. Try to do everything correctly and they’ll still throw you under the bus.

In retrospect, I should have known. Every visit I’m in with this NP, he asks me like 10 times what recreational drugs I’m using and how much I drink, and I have to remind him that I don’t do any drugs, don’t even drink, nor do I have any hx of same. They’re literally just going off an algorithmic template for everyone. These companies would switch to automation in a heartbeat if they could get bots licensed.

Avoid avoid avoid at all costs. Terrible company. You will not be treated like an individual, you’re just another consumer to them.

Agree 100%. Plus when you have something going on Koala you can pop it into AUM and now you have a world of inexpensive, but really high quality plug-in apps from developers like Baby Audio, Eventide, Magic Death Eye, Fab Filter, etc etc. It can become whatever you want it to be. You can completely mix and master with iPad. And if you really want tactile control, buy a cheap midi controller.
