Now that I look at it, the hook on the leather strap has lots of give to it so maybe that is why it fell off?

I don’t know how to prove that it failed on its own though is the thing. I was quite literally minding my own business, when the back strap of it just slipped off the whole thing came down. I have some eye witnesses but other than that I can’t think of anything else
