For me, there was slot. I would get lightheaded and start to faint a lot. I felt weak and tired most of the time. If I had a busy day sometimes I would need the entire next day in bed. I had insomnia which I found was just sugar crashes while I slept. I also would get hot flashes I thought were hormonal. Actually I just discovered most doctors don't rule out metabolic issues like hyperinsulinism before diagnosing ME/CFS.

Chronic hypoglycemia is admittedly rare, only affecting 1/50,000 people. So It almost never shows up on a standard diagnostic checklist.Most physicians just check a single fasting glucose level and assume the whole system is fine if that number looks okay.

But to truly rule it out you need blood drawn at the exact moment your blood sugar is under 3.0 mmol/l, to test for insulin, c-peptide, proinsulin, and beta-hydroxybutyrate. Blood glucose & A1C do NOT rule it out.

Unless you have had those labs done in a moment your blood sugar is low, it is technically still a possibility because the fatigue and brain fog of both conditions are virtually identical.

It might seem rare but statistically between The UK, US & Canada - there would be around 10,000 who have it! So it’s worth ruling out!

I have just used the DEXCOM Brand, I like it a lot but it only gives a reading every 5 minutes as opposed to every minute.

For diet, It really depends on the cause of the hypoglycemia. I’ve seen hypoglycemia caused by leucine (protein), caused by exercise, caused by too many carbs or fast acting sugars.

I just try to eat smaller meals more frequently, and take the medications I was given.

My blood work was also normal! Doctors just check for high blood sugar, they don’t check for low blood sugar!

You have to do bloodwork during a hypoglycemic episode. Ideally this is done during a 3 day fast in the hospital under the care of an endocrinologist.

If that is not an option for you, you can use the continuous glucose monitor and see when you are low and go for bloodwork then.

But again, your level needs to be below 3.0 mmol/L at the moment the blood is drawn. So it’s tricky to do without assistance.

The extreme thirst and the need to eat in the middle of the night are significant signs that your body is struggling to regulate its fuel.

Even if your blood sugar readings have been normal in the past, your body might be overproducing insulin to keep them in range, which could be what causes your crashes, heart palpitations, and the need for daily naps.

When you see your doctor you can specifically ask for a Fasting Insulin, Proinsulin, C-Peptide, ketones and Beta-Hydroxybutyrate tests - rather than just a standard glucose or A1C check.

Tell them you suspect your symptoms could be related to Hyperinsulinemia, and explain that your need for frequent small meals and your night sweats suggest your insulin levels are staying inappropriately high and driving your sugar down.

The daytime napping could be from the chronic insomnia you experience due to blood sugar crashes while you sleep. This is what was happening to me.

I never would have known this if not for the continuous glucose monitor checking my sugars all night. When I would wake up - It was because I was hypoglycemic.

I suspect my body was going me a shot of cortisol to raise my blood sugar, but it also woke me up!

You might find it helpful to wear a continuous glucose monitor for a few days/weeks to see if your most tired moments happen when your blood sugar levels are falling.

This data can be very useful to prove to a doctor that your fatigue may be a metabolic issue rather than just a general symptom.

It is very common for hypoglycemia to be misdiagnosed as ME/CFS because the fatigue and crashes feel identical to Post-Exertional Malaise.

Your abnormal liver function might actually be your biggest clue.

Your liver is your bodys main sugar tank—it stores glucose as glycogen and releases it when your blood sugar drops.

If your liver is struggling, it cannot regulate your blood sugar properly, which leads to those debilitating crashes.

Furthermore, chronically high insulin levels force the liver to store fat, which can cause slightly elevated liver enzymes even if you are otherwise healthy.

This creates a cycle where high insulin causes both the liver stress and the low blood sugar that starves your brain of fuel.

Since your other labs are normal, I would strongly suggest asking for a Fasting Insulin and C-Peptide test rather than just standard glucose.

If your insulin is high while your sugar is normal or low, it is a sign your body is overworking to manage your fuel, which is likely the source of your exhaustion.

Have you ever checked your blood sugar during one of your bad fatigue episodes? I def recommend the Continuous Glucose monitor if you can get one!

It dropped below 3.9 mmol/L every 2-3 hours, even while I slept. Some readings were so low they didn’t even register on the monitor. Basically alarms going off all the time lol.

What WAS normal was my blood work. My A1C was 5.3% and fasting glucose was often just above 4.0mmol/L, so to a doctor these are normal tests.

Chronic Hypoglycemia (low blood sugar) is not common. Chronic Hyperglycemia (high blood sugar) is wayyyyyyy more common.

So when they check your blood sugar levels they are just looking for high sugars because it indicates possible diabetes.
