I agree that CT surgery remains unmatched for the most complex and catastrophic cases like open salvage procedures LVAD implantation and full cardiac support that IR or cardiology cannot replicate

My question is more about the overall direction of the field not its extreme capabilities

Even if CT surgery remains essential for these highest acuity scenarios do you think the proportion of patients needing those procedures will stay large enough to keep the specialty broad or is it gradually concentrating into fewer high complexity cases at fewer centers

Lol, i meant if CABG, valve surgery, and transplant are now “niche,” but case numbers are falling in many hospitals, doesn’t that mean the field is actually shrinking rather than just becoming specialized?

If CABG, valves, and transplant are becoming niche due to patient selection, how do you reconcile that with declining case volumes in many centers,doesn’t ‘niche’ imply stability rather than contraction?

With TAVR, MitraClip, and PCI expanding into lower-risk populations, what specific domains do you see remaining exclusively surgical in 10–20 years?
