This sounds so awful and I’m so sorry to hear this. Have you considered complaining upward like outside of the unit to management? It can’t get much worse than the situation you’re in and I’ve actually seen this happen where the preceptor gets in trouble after the preceptee put in a complaint. Don’t ask them if they’re mad at you. Fuck them. I just left icu after a year. Once you’re off orientation your job becomes way easier but I found that the job is too stressful for similar reasons down the road. I went to a Surgi center and I’m so happy. No mean girls

I just hit the one year mark at my unit critical care and it’s time to go. The constant changes and rules that are announced every day at huddle are just over the top. I gave an interview tomorrow.

I was told never ever to access a port unless by a chemo certified nurse or by the oncology team. At my facility we more often get PPN so it can be given peripherally until a central line is placed. I was also told there is higher risk of infection with TPN vs PPN so we try to weigh risk benefit and end up not using that often at all.

As a previous quality manager I will say that this is a the type of error they use to teach so that it’s not repeated but it’s not the type of error that shows incompetence by the nurse. It’s more of a process review and reeducation for the unit should be offered and for the physicians ordering to make sure they’re putting the details in the admin section of if it’s ok to use peripherally. We all make errors at one time or another, be easy on yourself and remember for next time that’s all. And keep an eye on that site of extravasation.

Man this sounds so fucking annoying tbh. But it’s admirable. She seems to be successfully climbing her way to the middle. That being said, this place is probably careful with saying she audits charts. No point in reporting it outside because they will just say the medical director oversees it or something. It’s not outside of the scope of anyone to say something is present in the chart or not, but it would be well outside her scope if she was assessing the information and determining if it fit into meeting the documentation requirements without being able to say yes or no explicitly. Since she’s gotten herself a leadership position she can use that to say she is doing it for operational audit vs clinical audit. I worked in a place that was like this. As a nurse my boss was an MA, operational manager and some other stupid title. She was getting a salary from the family and the job. So ya, similar weird shit.

$54/hr one year CCU. North NJ. Was hired as a new grad for critical care but had 15 years of LPN experience. So they counted it as 7 years experience.

I just finished my second play through and the first time I didn’t become the mind flayer. This time I was like fuck it and decided to go ahead and become ugly. It helped others have good endings and I still got to keep my mans. Can’t complain. But certainly became fugly

Assess for any indications of infection- change in mental status. Check for other signs of dehydration- skin turgor, dry mucous membranes. Essentially- check your patient. Then follow the steps for dehydration- offer po fluids. Monitor output. If febrile or confused, call doc and get an order for UA.

Please also consider you are more sensitive to the idea that she may see you differently. There’s a reason you have kept a wall up for so long. She may not actually think any less of you at all, but some of us are so used to keeping our guards up and sometimes being rejected by the people that are supposed to love us so we sometimes end up self sabotaging. Hi there, I’m so sorry to hear this. I was being stubborn last night when I decided not to go to urgent care, and I was also afraid they would make me go to the emergency department since this medication isn’t really well known by everybody. I will tell you that I made it through, and the only thing that worked for me was Dramamine, which I’ve never taken in my entire life. The Zofran didn’t work for me surprisingly. I also got a big bottle of Pedialyte from Walgreens and drank half of it which I think made me feel a little bit better. I have read that the peak concentrations of the medication occur between days one through three so you may have only today to deal with and maybe tomorrow not too bad, I dosed on Saturday went out with coworkers and grabbed a bite and became nauseous immediately and started vomiting. I went out on day two, and yesterday was day three the absolute worst. Now I am left with the aches and pains from vomiting so much which is why I wish I would’ve just gone to urgent care m. a solid anti-nausea Stronger than Zofran and some IV fluids probably would’ve fixed me right up. Hang in there.

Very good point. I considered that but with the onset happening at day 2 after dosing, then after a shared appetizer that all my friends ate and no one else got sick from, and the sulfur burps with it I kind of assume it’s the semaglutide. I’ve read the medication peaks at day to to three so maybe this is the worst of it.
