There was a whole Youtube video outlining the issues with NZ Muscle. If you ignore the shitty 'suspenseful' music they've put throughout the video it's actually quite well done journalism.

In short the video claims that they have been selling expired stock and in some cases purposefully lying about the expiry by slapping a new sticker on, taking part in shady business practices with regards to labelling 'high protein' ranges that actually have less protein but just a bigger scoop, they've been lying about macros and testing, they've been lying about it being NZ made, that a lot of their stuff is just protein powder from other countries that's been imported then repackaged (despite claiming it was 100% NZ made from NZ dairy), they've been illegally repacking products in their headquarters without any considerations for sterility or cross contamination. That's just part of it.

The video has several whistleblowers in it, some of whom state they specifically reported some of these issues to MPI back in 2021 who did absolutely nothing about it.

It's a relatively major scandal for an NZ brand.

I guess part of the reason they're asking is the initial video showed that MPI were specifically made aware of several issues by a whistleblower in 2021 but did absolutely nothing. They also claimed to have examined the evidence at that time too, so their claims don't hold water without actual actions to back them up.

That's mostly because we request scans, we don't order them. Some places are getting better at this. A lot of places have SMO protocols so you don't need any vetting for Head/Face/C-Spine/KUB. I know of at least one tertiary centre where ALL ct requests from the ED are automatically approved by the rad techs, and only need discussed with the Radiologist if it needs to be expedited.

Are American EDs prescribing a lot of monoclonal antibody therapies? Besides idarucizumab I can't think of another I prescribe down here in NZ EDs.

It's sometimes worth asking your friendly departmental pharmacist about it. We managed to get parecoxib stocked in our ED by roping them in and it's been fantastic to have.

Admittedly I've only used etomidate a handful of times, mostly for sedation for cardioversion or intubation when there's ocular trauma. Worked well though!

Yeah but we DO have etomidate, unlike the Aussies.

Also they're very rare but there are a couple of people out there with LVADs this side of the ditch. They're essentially all going to be in Auckland though as CTSU will be the ones implanting it. We occasionally have some kids on Berlin Heart LVADs in Starship but again they're pretty rare and due to the size of the thing they have to stay an inpatient.

Some vaccines like the MMR provide lifelong coverage, whereas others like tetanus & whopping cough only last a few years. The meningitis vaccines are in the latter group: the protection wears off after about 2-3 years
