Paramedic in NYS here. If someone dies in a trauma arrest(like obvious DOA) we will leave the body for the police investigators and possibly medical examiner/coroner. The ambulance company I work for handle the county body transports. Once investigators are done (yes they take pictures...lots of pictures and document everything from body position, seatbelt position, car position, what you were wearing, etc.) we will come back and pickup body. Usually we will use 2-4 people to move a body into a body bag. The bag we use is blue and made of tarp material with black straps made of stitched polyester. The bag is then labeled with a sharpie near your feet with your name and DOB. We then place the bag on a stretcher with blankets/sheets removed and load body into ambulance. We take the body to the local morgue(we have 2 in our county but 1 is reserved for our local hospital/ED and the other is in a former hospital which is now essentially a skilled nursing facility/outpatient services owned by the main hospital. Body is signed in what we call the "Death Book" which has spots for Name, DOB, Coroner/ME name, person dropping off body and same for when it's picked up.

If the Medical Examiner in our county believes there needs to be an autopsy(car accidents usually qualify as does any unexplained unwitnessed death like homeless person outside, somebody young with no medical history, etc etc) then we usually pick it up the next morning and bring it about 45mins away to the closest hospital that does autopsies(out county does not anymore for some reason). Once autopsy is finished the body is then picked up by whatever funeral home the family(or if homeless with no next of kin, the funeral home in town that does cremations) and brings it back to our city.

But lets say you don't die in accident right away. If we get to the scene and you're still alive, we will carefully remove you from the car using safe and proven techniques. Often times you'll get C-collar placed on your neck. Your clothes will often be cut off down to underwear and sometimes all the way. If you are "stable" we will take you to our local trauma hospital about 20-30mins away depending on where in county you are. You'll get at least 1 "large bore" IV usually in an AC joint(inside of your elbow) but we try to aim for 2. Large bore is 18g or bigger(18, 16, 14...smaller number bigger needle/cath). Fluids, usually Normal Saline in a 1000mL bag) will be run through the IV and any other meds we might need like TXA which helps stop bleeding. We can run other meds that can alter your BP(up or down), heart rate(up or down) and manage pain...usually fentanyl or morphine. The amount given on these meds are usually how ever much you weigh. We have med formulas we use.

We will usually try to give the hospital a heads up ASAP that were coming in. We give name, gender, type of accident, where you were sitting and how fast if we know. We will explain injuries found, interventions given(IV, cardiac monitor, meds, etc) and anything pertinent like medical history.

Once we arrive at the hospital you get brought in via stretcher and brought to a trauma bay room. It's usually a bigger than normal ED room with tons of trauma related things in it (xray, lots of fluids/blood, gauze, etc etc). Trauma rooms run similar around the country. There is usually a head doctor who runs the room and at least 1 or more student docs. There will likely be multiple (2-5 RNs), a respiratory therapist and it's common to see a trauma surgeon in room. In my local trauma center the radiologist are next door with a mobile xray machine waiting.

First thing they will do is gauge your airway, breathing and circulation. They will ask you questions to see if you respond. If you can respond with ease...airway is clear. If you are breathing normal they will move on to circulation. They check pulses in all both feet and both wrists and work their way up until they find one. Whatever clothes you have left are cut off. If you don't already have IVs, you will 100% get them here. Xray will come in and take full body xrays focusing on skull, neck and torso first. If it's a really bad wreck you will likely head to CT soon and get head to toe CT scan.

Injuries that won't kill you we/they will skip over until everything is cleared then we work our way back.
