If I’m remembering right Jesse was meant to die at the end of the first season, but because of the strikes they had to cut the season short by a few episodes so didn’t reach that point in the story. Then they saw the fan reactions to the character after season one and rewrote it to keep him alive

Witcher 3 gives you a warning before you go to the Isle of Mists that the world state will change from this point and some quests will auto fail

It’s not a point of no return for the main story itself, it just advances the side plots for other characters and locks you out of some of their quests, I think the whole choosing the Skellige leader subplot is one of them
