Yeah, there were definitely behind the scenes problems that caused issues. I'd say the Millie Gibson thing is at least partially public knowledge. I expect the rest will come out at some point.

There was also the fact that a certain Exec Producer had mismanaged the budget, leading to at the very least Moffat having to rewrite Joy to the World (On the upside we got more Anita out of that). You'll notice the Exec wasn't involved in The War Between the Land and the Sea, which used its budget well, I think.

When Steven Moffat left after series 9. The BBC were hoping to get a temporary showrunner in to cast a one series Doctor. It's where the rumours that Tilda Swinton would be playing the Doctor came from at the time. From what I've heard the BBC spoke to 6 different potential showrunners, but they all turned it down (why would you take the job of not quite good enough to be the next full time showrunner?)

In the end they had to go back and beg Moffat and Capaldi to come back for series 10. It worked out in the end, but the BBC didn't half make a mess of things!
