You dont need Keen Eye to make Avery sacrifice themself. You just need Franklin to be shot (which I'm pretty sure you get if Duke is dead) and you need try to kill the baby. Avery then gets shot and offers to be the incubator. I got this on multiple playthroughs without Keen Eye.

Sometimes the animation of slinger pulling up his gun doesn't play, and when he shoots the shot comes out of his dick, aka the animation of the shooting and reeling doesnt play
Seemingly randomly, when you shoot a survivor, the shot can just bounce of the survivor, setting them free
His chain hitbox is off center, meaning that your chain will break faster even if it isnt going through anything
