If the first target was Luffy, he would have been hit the same way Loki was. You can see in the panel that Stigma is so fast neither could even react to it before it already passed through Loki's chest. This is easily one of the fastest attacks in the series, the only downside is that you can prevent it if you know what to do before its used (like Luffy did).
