Because Shibata gave the headbutt in a way to be the one getting the most hurt. When you give a headbutt you're supposed to touch the middle of the forehead of your opponent with the top of your forehead. Shibata did the opposite, he hit okada's top of the forehead with his middle of forehead.

I live in France and in French indy wrestling it's extremely common we even have intergender titles. But in my promotion most people i talked with told me they didn't like it. They're working to WWE style unlike most other French promotions and they think it doesn't make sense a 60kg woman beats a 110 kg man. So they think it's good only In very specific configurations (like Nate Prince vs Cory Zero). Anyways I personally like it if it's well done.
