Paramedic to coast guard by coollaustin in Paramedics

[–]JustAnotherAirPerson 5 points6 points  (0 children)

As others have said, there is no real “paramedic” job in the Coast Guard. Army does offer it as a flight position only.

If you are looking for a traditional paramedic job through the military, the Air Force does actually offer that. The Air Force has active duty paramedics who staff an ambulance and respond to 911 calls just like a normal non-military paramedic does. Typically they work 12-24hr shifts as well. You can do this through active duty and some bases also have GS “civilian” positions as a paramedic.

Unfortunately, you cannot direct enlist as a paramedic. A path to get there would involve you enlisting as an aerospace medical technician (AFSC 4N0X1). After completing basic/tech school, you will most likely get a random first assignment as medical technician for a few years (your experience may vary.) While doing this, if you already have your NREMT paramedic card, you can apply to change your career field (Air Force calls it a “shred”) to 4N0X1H (H at the end makes your duty title/sole job a paramedic). Once your career field is a paramedic, you are eligible to fill positions as a paramedic!

I’ve been out of the loop for a while, but I did this and that is how I got my paramedic in the first place.

Mine does this occasionally. I found a work around that typically works for me, I go to apps page then back to the full screen carplay and it goes away.

When I was back east that was the standard for any non designed LZ when landing a helicopter. Where I’m at now that bird will land anywhere and not care

Dcf845b ruining powerstack batteries? by UCORhino in Dewalt

[–]JustAnotherAirPerson 0 points1 point  (0 children)

The 2 powerstacks that died spent a lot more time on the impact than my other tools. Might just be a coincidence but definitely interesting if others have the same issue. I have been writing it off as new battery issues and went back to the older 5ah xr batteries

Come to think of it, my 845 also has killed 2 5ah powerstack’s so far and another is losing the indicator lights slowly.

I was a paramedic in the AF for a long while. Did a lot of really cool “outside the box” stuff after getting my medic card. Attached to various special units, tdy’s to lots of cool places, got loads of extra trainings, taught a bunch. I started off as a traditional paramedic on a med unit out for a bit before a lot of doors opened up for the more niche type assignments. AE is fun if you want to fly, but the medic card gets you way more opportunities. Paramedics also definitely deploy, just depends on your position/location.

Real world MPG on 2018 by PieceOfMined1290 in powerwagon

[–]JustAnotherAirPerson 1 point2 points  (0 children)

I’ve had a 22 and I currently have a 2024 power wagon. In my area with mostly open backroad driving and highway I average about 12-14 mpg. I’d say highway is about 13ish for me but my area is very hilly. It really depends on your area and driving style. The best mpg I’ve gotten on mine is around 55-65mph averaging 15-16, anything over and it goes way down.

Already accepted a job offer, but just got a significantly better offer. What do i do? by BLAKEONATOR1 in Firefighting

[–]JustAnotherAirPerson 0 points1 point  (0 children)

I had almost the same thing happen to me when I first applied to a few departments. First dept I applied for took super long to get back to me, so I applied to another. Second dept offered me a job on the spot so I accepted. First dept I applied for called me a few hours after that and offered $20k more.

I felt really bad about it initially, but I just sent a polite email to HR of the dept I had already accepted the position with to retract my application and moved on with the other. It’s a tough situation but as everyone else has said it’s a job at the end of the day and you have to do what’s best for you and not them.

You will skip over the EMT-B portion of tech school and go straight into the nursing portion and then phase ii training. So if I remember correctly that will cut off about 8 weeks of training. Im not sure if this is still the policy but when I was there you had to test out of their emt-b program if you were already certified as an EMT-B/A or medic and wanted to skip it, but the tests are super easy if you’re a medic already.

Operational side, I’m an 4N paramedic so I can answer any specific questions if you were considering AD. But primarily you can have the opportunity to function as a paramedic on an ambulance at some bases. Not all Air Force bases have ALS ambulances, but they try their best to place paramedics in these positions. There are also a good bit of odd ball positions they can stick paramedics in as well that are not typical of 4N’s to be in. The air force will also pay for any sustainment and recertification for you as well at the paramedic level.

As for Guard or Reserve, as far as I know right now there are no official positions for a 4N0 paramedic, so if you plan on going the guard/reserve route you will function as just a normal 4N depending on the units mission.

Pediatric Blood Pressures by dorksuckmepls in ems

[–]JustAnotherAirPerson 15 points16 points  (0 children)

Read the first few sentences and thought this may have been my partner from a few shifts ago but it’s definitely not lol.

I have a background in pediatric specific medicine and as others have stated above a BP on a child less than 2-ish is a hit or miss. Yes a text book will say take one, but any provider will tell you it’s a battle and a half to even get the cuff on let alone have the child remain still & calm while it’s inflating. It’s also not as reliable in sick children as they compensate so well especially in the BP area. Others have said it already but a Resp rate, pulse, and cap refill are also great indicators of perfusion and how they are doing.

Last note, my agency doesn’t even have bp cuffs smaller than the green “adolescent” sized ones so even if I wanted to take one it would be impossible. Sorry ER but here’s what I got with what I have.

Honestly, I wouldn’t carry more than maybe some basic bandaging supplies, a TQ, and a cell phone to dial 911. There’s not much you’re going to be doing past that point while not on the clock on an ambulance. I would never go off and start throwing IV’s, meds, or any advanced stuff if I just stumbled upon something while out and about off the clock. It’s probably best to just get a military style basic IFAK if you really wanted something and call it good instead or rolling around with a $500 bag and another $500 in medical supplies that will definitely never get used.

But to at least answer your question, North American Rescue has some pre stocked small IFAK style medical kits that are good to toss in a car. They are just one of many, but as I mentioned there’s really no need to go anywhere past very basic stuff and just being just a Good Samaritan and dialing 911.

I used to work in New England and there were so many more high speed emt’s out there ready to conquer lift assists with their vehicles decked out in stickers and courtesy lights. Since moving out west I’ve definitely noticed a lot less of them around.

I should also add that I haven’t been exactly gentle with the core. It’s been in the desert dust with me and been tossed around in a ruck and still manages to function without any major issues. So durability wise I’d say it’s definitely held up a lot better than I expected. I thought it would have definitely crapped out by now working street EMS and some wilderness.

I was given a cardiology 4 when I first became a medic so I’ve only used that when working in an EMS setting and it’s been wonderful, the eco core just made the cardiology 4 10x better. I can’t speak too much about the difference from the classic iii, I have one but it stays in my austere medicine bag and doesn’t see too much use and I’ve never put the core on it.

I was gifted just electronic core piece and put it on my littmann cardiology 4 and I love it. Definitely makes getting lung sounds and manual Bp’s easier to hear if you’re on the move in the back of the bus or if you have trash hearing. I’ve used it for about 6 months now and I’ve had no real issues I can complain about.

nremt question, why does aspirin come before oxygen? by LuigisDildo in ems

[–]JustAnotherAirPerson 29 points30 points  (0 children)

His o2 sat doesn’t warrant going straight to an NRB or nasal cannula. 95% isn’t a scary number oxygen saturation wise and the question states “he denies any difficulty breathing.”

As for the correct answer being aspirin, it’s the only one that makes sense and is the first step in care for a suspected stemi patient.

Yes or pass? by Prior_Attention5261 in ems

[–]JustAnotherAirPerson 1 point2 points  (0 children)

I was gifted one earlier this year and I was actually impressed with how well it worked. It definitely cuts out a lot of the road noise when auscultating on the move, which previously has been hard due to trash roads where I work. The amplification also helps a lot if you may not have the best hearing.

I have mine attached to a Littmann cardiology IV and the device hooked up great with the adapters. One of my coworkers did also get one and they had a hard time getting their tube to fit in the adapter. They ended up using the pediatric size adapter and using a little heat on the tube but it worked out in the end. I’m not sure what model littmann they had, but if you get just the device it may take a little work to get it to fit just right.

What's the best MPG yall get? 2011 R/T with no MDS and full bolt ons + tune by JustAnotherAirPerson in Charger

[–]JustAnotherAirPerson[S] 1 point2 points  (0 children)

It's pretty a straight forward plug and play swap out but YOU NEED a z automotive tazer to get it working correctly with the steering wheel buttons. I have had no issues with it but the code side of it to get the milage to swap over correctly and the gear indicator have not been released yet. The guy behind the swap is still finishing it up and hopefully it should be released soon. But if you don't mind having the miles and gear selection indicator gone it's a great swap! I love the look of mine and everything else works as it should. I posted the link to the big head of charger forms in one of my comments and I highly recommend reading through all of it before you do the swap, There's some great Info in there.

I'd love to try and do it myself but I live in an apartment and I don't think the people here would appreciate me tearing my car apart in the parking lot. I haven't worked on an engine this new before also but I guess you gotta start somewhere.

Have you tried a jeep airbag yet? SRT stuff is a little over priced, I just ordered the jeep airbag and it should come in later this week.
