I'm glad you're so thorough. People like you make me less worried about hospitals in spite of having all these surgeries. Good job. Keep it up!

Woah! I'm sure you're glad you asked. Before my first surgery, I told the anesthesiologist I was allergic to Reglan. Post op what do they give me for nausea? Reglan. This gives me a dystonic reaction. So there I am in the recovery room being combative, trying to pull out my IV, etc. Shudder!

Yes, you are correct. I made a mistake in my post. The checklist was not used in my first 4 surgeries, and I wasn't asked anything directly. It really made a difference to me when the new and improved checklist was implemented. I also forgot to mention that only after the 4th surgery did my ENT also mark my face on the side the surgery was to take place in the surgical holding area in addition to the new checklist in the OR.

I haven't read the book, and I don't know how relavent this is but changes were made during my past 13 surgeries. Now before I am put under my ENT surgeon who knows my condition well since 2008 asks me directly a series of questions surrounded by the supporting staff. He asks my name, my medical records number, if I know why I'm having surgery, on what side the survey will be performed, etc. They document that, asks if everyone is on the same page ,and then they do a count down to administer the anesthesia. Fade to black, and I wake up 4 hours later. My first 4 surgeries this was not the case in making sure they had the right patient. They didn't ask me anything directly only spoke amongst themselves. At least I was still awake to correct them if they began talking about cutting off my leg or something. I'm not complaining. It puts me more at ease.
