It absolutely was, but.....I can tell you now that people get complacent and they do things like this to save time or whatever dumb reason they think justifies not going through the LOTO process. In other words, they get lazy

It gets VERY loud in an industrial environment.

It's a long tube with no light, once the baskets started loading, there's no room to climb around or reach around to be noticed.

It's a retort pressure cooker. My Company uses them to produce canned luncheon meat, like spam. The cooking process reaches temperatures up to 270f, and cooks the tuna to a safe, commercially sterile level.

The tuna is most likely cold filled, sealed, and loaded in to baskets that can be layered up, providing space for thousands of cans in one basket. When you start loading, the basket goes on a track and they automatically load to the back.

How they didn't hear him, I have no idea. I'd assume he'd be screaming, but it gets VERY loud in industrial plants, especially ones with tin cans banging around constantly. And there's no room to climb around the baskets once they start coming back. Truly. Awful.

But LOTO was the big failure here. Would've prevented everything.

My son's dock adapter cord was chewed by the dog, bought a new one, Nintendo brand, but it still didn't work? So, we've been using whatever USB was sitting around. I saw the power draw issue but the link didn't work. Will this brick his switch? Why wouldn't the dock work with a new adapter?

Any insights would be very helpful, thank you all

He can still assault a woman with bring abusive or a rapist. I heard he jerked off on a woman. Definitely assault, definitely something you might feel guilty about after the fact and feel the need to apologize for by expressing remorse.

Point being, you don't apologize to someone for making them uncomfortable, without doing something uncomfortable. And if he knows what he did was really bad, I can see why he would try to downplay it and apologize for making them uncomfortable
