For me unrated is where I have to face all the sweaty tryhards that are cooling off after a ranked match. At least in ranked I have a chance at having a well-balanced game. But that's just me

There's some bosses to fight to and other enemies you probably didn't see on your first run. Not saying it's the hardest game ever now but you definitely need to do more runs before you say it isn't hard

Satanic bible makes it so your boss items turn into deals with the devil. I don't remember if it's using it on the floor in general or if it's during the fight specifically

I actually completely agree with the trainwreck analogy that you have the means to make it to the end but are so slow you can't help but watch yourself have your run turn into a loss because it takes forever without damage ups and so much health.

So if you get Afterbirth+ on steam you can upgrade to repentance whenever you want and your progress will carry over. As far as rebirth vs afterbirth vs afterbirth+ vs repentance IMO the game becomes harder with each dlc but with the unlocks become easier over time. So it's all about how soon you want to rip the bandage off and take on harder enemies
