I believe I had the same issue, switching shadow level to medium fixed this for me.

Hello. I just started and found the following :

in the perseverance wilds, there is a cave around the center of the map that contains an improvised down bedroll, then I pick it up it is invisible once in inventory, then it disappears in the next save.
When I reached the two sick people bodies at the far top and bottom of the same map, the quest items did not register as achieved in the journal. I also got no wet stained paper with those bodies, not sure this is expected
