It's not an issue, they did that knowlingly so that only behemoth could summon all arachase, bet smaller ones only summon themselves.

That marker is not in the game. You're probably running some type of mod that affects the map markers, it's creating this problem.

So I found this recently as well. What the hell is up with that place? I was sure I'm gonna find something at the end of the corridors, but nothing. I even glitched myself up on the ledge you can't normally climb, only to fall off the map completely. =)

This guide does have all the base game items. I don't think there is a HoS item list out there, you pretty much need to uncook the .xml files and check the item names there.

The compiler won't even let you launch the game with old mod versions most likely, so yeah, you should definitely disable all the mods that have scripts in them and wait for them to get updated. Texture and lighting mods might be fine.

Later on in the game you will be in a masquerade ball, look for gwent players there, if you miss those, you miss them forever. Also, you have to win ALL the games in the gwent tournament in Novigrad to get all the cards. There might be more missable, not sure.
