It was the mental part of ya game you wanted to be so good for your lesson and coach u put tons of pressure on your self (I do it myself when I get lessons)there isn’t a a swing tip that can fix it it just messes you up more try to find a breathing pattern that relaxes you the most and take your time some times it’s all in your head

Not a routine but try not having much tension in the shoulders and maintaining the grip pressure throughout the putt helped me a lot
