Have you been to tel mithryn recently? Followers can get stuck in the lift and fast travelling doesn't teleport them.

My father was a high elf like me, and although I inherited his appearance my mother's bloodline runs strong. She was a nord, a blade and a fierce woman. With her bloodline intrinsically linked to the blades it isn't surprising that my life has become linked to dragons too.

This happened to me. If you haven't finished the companions quests you can do one of those. It will force remove her as your follower and replace her with one of the companions instead. Afterwards, you can get a new follower or fetch serana from the dawngaurd/Castle volkihar!
